Nasopharyngeal carcinoma?
I would like to ask Dr.
Lin what symptoms can occur with nasopharyngeal carcinoma? My husband has been experiencing discomfort in his throat since August 2004, often feeling a sharp pain.
He has to constantly swallow saliva to feel somewhat better and frequently uses throat lozenges to alleviate the throat pain.
He has visited many hospitals but has not found a cause.
Recently, he noticed a tingling sensation in his left cheek.
He has quit smoking and has not consumed alcohol for nearly two years.
Should he undergo a biopsy? Please provide your insights.
Thank you.
Lisha, 30~39 year old female. Ask Date: 2004/05/24
Dr. Lin Zongzhe reply Oncology
Hello: The more common symptoms of nasopharyngeal carcinoma include nasal congestion, nosebleeds, tinnitus, neck masses, and cranial nerve dysfunction.
Your husband's symptoms could have many possible causes.
If the clinician suspects this based on a comprehensive assessment of clinical symptoms and examinations, then a biopsy is indeed necessary.
Sincerely, Dr.
Lin Tsung-Che, Oncology Department, Taoyuan Hospital, Ministry of Health and Welfare.

More Info
Nasopharyngeal cancer (NPC) is a type of head and neck cancer that originates in the nasopharynx, the area behind the nose and above the back of the throat. Understanding the symptoms associated with NPC is crucial for early detection and treatment. Here’s a comprehensive overview of the symptoms, potential diagnostic steps, and considerations for patients experiencing these issues.
Common Symptoms of Nasopharyngeal Cancer
1. Persistent Sore Throat: One of the earliest symptoms of NPC is a chronic sore throat that does not improve with standard treatments. Patients often describe it as a persistent discomfort or pain in the throat.
2. Difficulty Swallowing (Dysphagia): As the tumor grows, it can obstruct the throat, making swallowing painful or difficult. Patients may feel like food is getting stuck or may experience significant discomfort while eating.
3. Ear Symptoms: NPC can lead to fluid buildup in the middle ear, resulting in hearing loss, a feeling of fullness in the ear, or ear pain. Tinnitus (ringing in the ears) may also occur.
4. Nasal Symptoms: Patients might experience nasal congestion, frequent nosebleeds, or a persistent runny nose. These symptoms can often be mistaken for allergies or sinus infections.
5. Facial Pain or Numbness: As the cancer progresses, it may affect nearby structures, leading to facial pain, numbness, or weakness, particularly on one side of the face. This can be concerning, as it may indicate nerve involvement.
6. Lumps in the Neck: Swollen lymph nodes in the neck are a common sign of NPC. These lumps may be painless and can vary in size. They are often the first noticeable symptom for many patients.
7. Changes in Voice: Patients may notice changes in their voice, such as hoarseness or a nasal quality, due to the tumor's impact on the vocal cords or surrounding structures.
8. Weight Loss: Unintentional weight loss can occur due to difficulty swallowing and changes in appetite.
Diagnostic Considerations
Given your husband's symptoms, particularly the persistent throat discomfort and the recent onset of facial numbness, it is advisable to pursue further diagnostic evaluations. A biopsy is often necessary to confirm the presence of cancer. Here are some steps that may be taken:
1. Consultation with an ENT Specialist: An ear, nose, and throat (ENT) specialist can perform a thorough examination of the throat and nasopharynx, often using a nasopharyngoscope to visualize the area.
2. Imaging Studies: CT or MRI scans can help assess the extent of the disease and check for lymph node involvement or metastasis.
3. Biopsy: If a suspicious lesion is found, a biopsy will be necessary to determine whether it is cancerous. This can be done through various methods, including fine needle aspiration or direct excision.
Importance of Early Detection
Early detection of nasopharyngeal cancer significantly improves treatment outcomes. If your husband has been experiencing these symptoms for an extended period without a clear diagnosis, it is crucial to advocate for further testing. Given the potential for NPC to affect nearby structures and the symptoms he is experiencing, timely intervention is essential.
Conclusion
In summary, nasopharyngeal cancer can present with a variety of symptoms, including persistent sore throat, difficulty swallowing, ear symptoms, and facial pain or numbness. Given your husband's ongoing symptoms and the recent development of facial numbness, it is advisable to pursue further evaluation, including a biopsy, to rule out or confirm the presence of cancer. Early diagnosis and treatment are key to managing this condition effectively. Please ensure that you communicate openly with your healthcare provider about your concerns and the need for comprehensive evaluation.
