Testicular pain
I am 19 years old, and today I suddenly felt pain in my right testicle.
When I got home to take a shower, I noticed a movable, soft mass about 1.5 mm in diameter attached above my right testicle, which is also painful.
I'm concerned about whether this is serious.
Could it be epididymitis? Please advise, thank you.

Dr. Liu Jianting reply Urology
Sudden testicular pain at your age raises concerns primarily for testicular torsion, followed by infection or inflammation (which may cause fever and swelling).
Please consult a urologist for an examination!

More Info
Testicular pain in young men can be concerning and may arise from various causes. Given your description of sudden pain in the right testicle, along with the presence of a soft, movable lump above the testicle, there are several potential explanations that should be considered.
One common cause of testicular pain in young men is epididymitis, which is the inflammation of the epididymis, a coiled tube located at the back of the testicle that stores and matures sperm. Epididymitis can be caused by infections, including sexually transmitted infections (STIs) such as chlamydia or gonorrhea, or by non-infectious factors such as trauma or a reaction to certain medications. Symptoms typically include pain and swelling in the affected testicle, and it may be accompanied by fever or discharge from the urethra.
Another possibility is a testicular torsion, which is a medical emergency that occurs when the spermatic cord becomes twisted, cutting off blood supply to the testicle. This condition usually presents with sudden, severe pain and swelling in the testicle and requires immediate medical intervention to prevent permanent damage.
The lump you described could also be a benign condition such as a spermatocele (a cyst that forms in the epididymis) or a varicocele (enlarged veins within the scrotum). These conditions are generally not serious but can cause discomfort and may require monitoring or treatment if they become symptomatic.
Given your symptoms, it is crucial to seek medical attention promptly. A healthcare provider will likely perform a physical examination and may order an ultrasound of the scrotum to assess the blood flow to the testicle and to evaluate the lump. This imaging can help differentiate between conditions like epididymitis, testicular torsion, or other masses.
In summary, while there are several potential causes for your testicular pain and the lump you have noticed, it is essential to consult a healthcare professional for an accurate diagnosis and appropriate treatment. Early intervention is key, especially in cases of testicular torsion, where time is critical to preserving testicular function. Please do not hesitate to reach out to a urologist or your primary care physician for further evaluation.
