Temporomandibular joint issues?

Doctor, I have been struggling with temporomandibular joint (TMJ) disorder for four months.
For the first three months, I only experienced difficulty opening my mouth without any noise, so I underwent conservative treatment.
However, in the past month, I have frequently heard noises from the joint.
Next Tuesday, I will be going to the oral surgery department at Shin Kong Hospital for splint therapy.
I would like to ask if it is still appropriate to start splint therapy at this point.
Currently, I can open my mouth enough to fit three fingers, and I have no difficulty eating or speaking.
There is slight pain when I open my mouth wide, and I have mild inflammation in the TMJ, along with frequent noises from the joint.
I am very concerned about the possibility of disc displacement or perforation.
Could you please explain what the symptoms would be if there is disc displacement or perforation?

Hello, A-Chung.
The treatment for temporomandibular joint (TMJ) syndrome in the early stages, when there are no noises, typically involves medication.
When pain intensifies and there is difficulty opening the mouth, the treatment phase progresses to the use of an occlusal splint.
Therefore, your current treatment is indeed at the stage of using an occlusal splint.
Usually, if there is a displacement or perforation of the articular disc, the symptoms are quite similar to what you are experiencing now, but with more difficulty in opening the mouth, increased pain, and more pronounced sounds.
This can only be determined by the physician through clinical follow-up and some imaging studies.
At this stage, you can rest assured and follow your physician's instructions for treatment for a while.
If you gradually see improvement, there is no need to worry too much.

Jaw joint disorders, commonly referred to as temporomandibular joint (TMJ) disorders, can manifest in various ways, and your symptoms align with some of the common presentations of this condition. The TMJ connects your jawbone to your skull and is responsible for the movement of your jaw. Disorders in this area can lead to pain, discomfort, and functional limitations.

Symptoms of TMJ Disorders
The symptoms you are experiencing, such as difficulty opening your mouth and the presence of joint noises, are typical indicators of TMJ disorders. Here are some common symptoms associated with TMJ disorders:
1. Pain: This can occur in the jaw, neck, and shoulders. You mentioned experiencing mild pain when opening your mouth, which is a common symptom.

2. Limited Jaw Movement: Difficulty in opening your mouth fully, which you noted as being able to fit three fingers, can indicate a restriction in the range of motion.

3. Clicking or Popping Sounds: The noises you hear when moving your jaw may suggest that the disc within the joint is not functioning properly, which can occur due to displacement or degeneration.

4. Swelling: Mild inflammation in the joint area, as you described, can also be a sign of TMJ disorders.

5. Headaches: Many individuals with TMJ disorders report tension-type headaches or migraines.

6. Ear Symptoms: Some may experience earaches or a sensation of fullness in the ears, although there is no actual ear infection.

Treatment Options
You mentioned that you are scheduled for a bite splint treatment at the oral surgery department. This is a common and effective conservative treatment for TMJ disorders. Bite splints, also known as occlusal splints or night guards, help to:
- Reduce Muscle Tension: They can help relax the muscles around the jaw, reducing pain and discomfort.

- Prevent Teeth Grinding: If you grind your teeth (bruxism), a bite splint can protect your teeth and reduce strain on the TMJ.

- Improve Jaw Alignment: They can help in realigning the jaw, which may alleviate some of the symptoms.

Is It Too Late for Bite Splint Treatment?
Given your current symptoms, it is not too late to pursue bite splint therapy. In fact, starting this treatment can be beneficial in managing your symptoms and preventing further complications. The fact that you are still able to open your mouth without significant difficulty and that you have not experienced severe pain suggests that your condition may still be manageable with conservative treatment.

Concerns About Disc Displacement or Damage
You expressed concern about the possibility of disc displacement or perforation. If the disc in the TMJ is displaced, symptoms may include:
- Increased Pain: Pain may become more severe and persistent.

- More Pronounced Clicking or Popping: The sounds may become more frequent or louder.

- Locking of the Jaw: In some cases, the jaw may lock in an open or closed position.

- Worsening Range of Motion: You may find it increasingly difficult to open your mouth or chew.

If you experience any of these symptoms, it is essential to follow up with your healthcare provider for further evaluation. Imaging studies, such as MRI, can help assess the condition of the joint and the disc.

Conclusion
In summary, TMJ disorders can significantly impact your quality of life, but conservative treatments like bite splints can be effective in managing symptoms. It is crucial to monitor your symptoms and maintain open communication with your healthcare provider. If your symptoms worsen or new symptoms arise, do not hesitate to seek further evaluation. Early intervention can often prevent more severe complications and improve your overall well-being.
