A bunch of white granules?

Hello doctor, about two years ago and continuing until now, I have noticed small clusters of white bumps on my foreskin.
There is also a line of similar-sized bumps, about the size of a needle, in the groove behind the glans.
They don't look like blisters that would burst, and sometimes they itch, but they don't hurt.
What could this be? I have not engaged in sexual intercourse.

Xiao Hei, 10~19 year old female. Ask Date: 2004/07/13

Dr. Liu Jianting reply Urology

It is likely a papillomatous hyperplasia, which is harmless.
Please visit a hospital to consult a urologist!

Understanding white bumps on the penis can be concerning, especially when they appear suddenly or change in appearance. Based on your description, it sounds like you are experiencing small white bumps on the foreskin and a line of similar bumps behind the glans (the head of the penis). These bumps can have various causes, and it’s essential to understand them to determine if any treatment is necessary.

One common cause of white bumps on the penis is Pearly Penile Papules. These are small, benign growths that typically appear around the corona (the ridge of the glans) and are completely normal anatomical variations. They are not sexually transmitted and do not require treatment unless they cause psychological distress.

Another possibility is Fordyce Spots, which are small, painless, pale bumps that can appear on the shaft of the penis. These are sebaceous glands that are visible due to their proximity to the surface of the skin and are also harmless.

If the bumps are itchy, it could indicate a condition such as Lichen Simplex Chronicus, which is a result of chronic irritation or scratching. This condition can lead to thickened, itchy skin and may require treatment to alleviate the symptoms.

Another potential cause could be Folliculitis, which is inflammation of the hair follicles that can occur anywhere on the body, including the genital area. This condition can present as small red or white bumps and may be itchy or tender.

In some cases, Genital Warts, caused by the human papillomavirus (HPV), can appear as small, flesh-colored or white bumps. These are typically painless but can be itchy. Since you mentioned that you have not engaged in sexual activity, the likelihood of this being the cause is lower, but it is still worth mentioning.

If the bumps are persistent, changing in appearance, or associated with other symptoms such as pain, discharge, or bleeding, it is crucial to seek medical advice. A healthcare professional can perform a physical examination and may recommend tests to rule out any sexually transmitted infections or other conditions.

In summary, while many causes of white bumps on the penis are benign and do not require treatment, it is essential to monitor the situation. If you experience any changes or additional symptoms, consulting a healthcare provider is the best course of action. They can provide a definitive diagnosis and appropriate management options tailored to your specific situation. Remember, maintaining good hygiene and avoiding irritation in the genital area can help prevent some conditions.
