Varicocele Surgery: Outpatient Options and Recovery - Urology

May I ask the doctor?

Hello, doctor.
I have an issue with varicocele.
Can I have the surgery done immediately during an outpatient visit, or is hospitalization required? Since I am an adult, I would like to consult on my own and fill out the surgical consent form myself.
I also prefer not to let my family know about the surgery.
Can I request the doctor not to admit me to the hospital? Additionally, my left testicle is slightly atrophied; will it return to its normal size after the surgery? Thank you.

pp, 20~29 year old female. Ask Date: 2004/07/21

Dr. Chen Xiuyi reply Urology

Hello: In cases of varicocele, if sperm quality and quantity are affected or if varicocele causes testicular pain, surgical treatment should be considered.
Generally, the surgery requires spinal anesthesia, which necessitates hospitalization and consent from a family member for the procedure.
If varicocele has already led to testicular atrophy, some patients may not regain normal size after surgery; however, the quality and motility of sperm are likely to improve.
Approximately 10% of patients may experience recurrence after the surgery.
Sincerely, Dr.

When it comes to varicocele surgery, there are several important factors to consider regarding outpatient options, recovery, and the potential impact on testicular size.
Outpatient Surgery and Hospital Stay:
Varicocele repair can often be performed as an outpatient procedure, meaning that you may not need to stay overnight in the hospital. Many surgeons utilize minimally invasive techniques, such as laparoscopic surgery or percutaneous embolization, which can be done in an outpatient setting. These methods typically involve smaller incisions and less recovery time compared to traditional open surgery.
If you are an adult and wish to manage your own medical decisions, including signing consent forms, you can certainly discuss this with your surgeon. Most healthcare providers respect patient autonomy and confidentiality, so you can request that your family not be informed about your surgery if that is your preference. However, it is essential to ensure that you have someone to assist you post-operatively, as you may experience some discomfort or need help with transportation home.

Recovery Process:
The recovery from varicocele surgery is generally quick, especially with minimally invasive techniques. Most patients can return to normal activities within a few days, although it is advisable to avoid strenuous activities or heavy lifting for a couple of weeks. Pain management is typically straightforward, with over-the-counter pain relievers being sufficient for most individuals.
Impact on Testicular Size:
Regarding your concern about testicular atrophy (shrinkage) on the left side, it is important to note that varicocele can lead to reduced blood flow and oxygen to the affected testicle, potentially causing size reduction. After surgery, many patients experience improvement in testicular size and function, but results can vary. Some studies suggest that testicular volume may increase post-surgery, especially if the varicocele was contributing to the atrophy. However, the extent of recovery in size can depend on the duration and severity of the varicocele before surgery.

Conclusion:
In summary, varicocele surgery can often be performed as an outpatient procedure, allowing for a quick recovery without the need for an overnight hospital stay. You have the right to manage your medical care independently, including the decision to keep your surgery private. While many patients see improvements in testicular size post-surgery, individual results can vary based on several factors. It is crucial to have an open discussion with your surgeon about your concerns and preferences to ensure that you receive the best possible care tailored to your needs.

How effective is oral medication? You won't know until you have tested it yourself. In principle, you can be discharged one or two days after surgery. For detailed information, it's best to discuss it directly with the attending surgeon at the time.
