Struggling with Enlarged Pores: Seeking Solutions for Acne Scars - Dermatology

Please help me..?

Hello Dr.
Cheng: I previously heard that applying Metronidazole cream on my face every day could cure acne and blackheads, so I started applying it about 3 to 4 times daily.
Within less than a week, my pores became very enlarged, and by the time I noticed, it was already too late.
My pores are now excessively large, and I am very anxious.
I have spent a lot of money trying various treatments such as glycolic acid peels, laser hair removal, electrotherapy, intense pulsed light, photodynamic therapy, and diamond microdermabrasion.
I have also been very particular about skincare products.
I have consulted many doctors and used numerous medications, even hormonal treatments.
I have truly spent a lot of time and money, but nothing has worked.
Recently, a doctor told me that no matter what I do, it won't help.
That doctor also mentioned that having large pores isn't necessarily a bad thing.
I feel really sad; I already don't look good, and now I feel even worse...
I have completely lost confidence.
Dr.
Cheng, am I really out of options? If there are any other methods, please let me know.
I would really appreciate it.

Hello: If you have tried many methods and the size of your pores remains unchanged, there may not be any effective solutions available at this time.
If you are very concerned about this issue, you might consider using skincare products that contain silicone powder, which can quickly conceal imperfections, but this is only a temporary effect.
Sincerely, Dr.
Cheng Li-Chen, Dermatology Department, Taoyuan Hospital, Ministry of Health and Welfare.

Hello! I understand that you're feeling quite distressed about your enlarged pores and acne scars, especially after trying various treatments without satisfactory results. Let's break down the situation and explore some potential solutions.

Firstly, it's important to understand that enlarged pores can be a common issue, particularly for individuals with oily skin or those who have experienced acne. Pores can become enlarged due to a variety of factors, including excess oil production, dead skin cell buildup, and inflammation from acne. When pores are clogged, they can stretch and appear larger over time.

You mentioned that you have tried several treatments, including chemical peels, laser treatments, and various skincare products. While these treatments can be effective for some individuals, they may not work for everyone, and results can vary based on skin type and the severity of the condition. It's also crucial to ensure that any treatments you pursue are appropriate for your skin type and concerns.

Here are some suggestions that may help you manage enlarged pores and improve the appearance of your skin:
1. Gentle Cleansing: Use a gentle, non-comedogenic cleanser to remove excess oil and impurities without stripping your skin of its natural moisture. Avoid harsh scrubs that can irritate the skin and potentially worsen the appearance of pores.

2. Exfoliation: Regular exfoliation can help remove dead skin cells and prevent clogged pores. Consider using products containing salicylic acid or glycolic acid, which can penetrate the pores and help keep them clear. However, be cautious not to over-exfoliate, as this can lead to irritation.

3. Moisturizing: Even if you have oily skin, it's essential to use a lightweight, oil-free moisturizer. This helps maintain your skin's hydration balance and can prevent the overproduction of oil, which can contribute to enlarged pores.

4. Topical Treatments: Products containing retinoids can be beneficial for improving skin texture and reducing the appearance of pores. Retinoids promote cell turnover and can help minimize the appearance of acne scars over time.

5. Professional Treatments: If you haven't already, consider consulting with a dermatologist or a licensed skincare professional who can assess your skin and recommend tailored treatments. Options may include microneedling, chemical peels, or laser resurfacing, which can help improve skin texture and reduce the appearance of pores and scars.

6. Sun Protection: Always apply sunscreen daily, as sun damage can exacerbate skin issues and lead to premature aging. Look for a broad-spectrum sunscreen with at least SPF 30.

7. Lifestyle Factors: Maintain a healthy diet, stay hydrated, and manage stress, as these factors can influence skin health. Avoiding smoking and excessive alcohol consumption can also benefit your skin.

8. Patience and Realistic Expectations: It's important to have realistic expectations regarding the results of any treatment. Skin improvement takes time, and while some treatments may show results quickly, others may require consistent use over several weeks or months.

Lastly, if you feel overwhelmed or discouraged, consider seeking support from a mental health professional. Sometimes, the emotional toll of skin issues can be significant, and talking to someone can help you cope with these feelings.

In conclusion, while it may feel like you've exhausted all options, there are still avenues to explore. Consulting with a dermatologist for a personalized treatment plan can be a crucial step in addressing your concerns. Remember, you're not alone in this journey, and with the right approach, you can achieve improvements in your skin's appearance.
