Medication Consultation
Hello Dr.
Hou: I have been troubled by constipation for over a year (it's quite frustrating).
I have undergone a colonoscopy and X-ray gastrointestinal examinations at the hospital, and everything came back normal.
The doctor mentioned that my intestines are too long.
I have also consulted traditional Chinese medicine, but there has been no improvement.
I have tried various methods to alleviate constipation (such as drinking more water, eating more vegetables and fruits, exercising, etc.), but nothing has worked.
Therefore, I have to take medication to have a bowel movement; without it, I cannot pass stool.
I would like to know if long-term use of sennosides will have side effects.
Additionally, what side effects might powdered rhubarb and powdered aloe have? Thank you.

Dr. Hou Fangmin reply Pharmacology
Hello: Many people experience the issue of constipation.
The Sennoside you are taking is used to treat constipation and has relatively few side effects; it is one of the medications with the least side effects among all treatments for constipation.
As for the rhubarb and aloe vera you mentioned, they are commonly used traditional Chinese medicine laxatives.
If you plan to use traditional Chinese medicine as a laxative for an extended period, it is advisable not to self-medicate but rather to consult a qualified traditional Chinese medicine practitioner.
They can assess your body condition and adjust your medication accordingly.
I hope this response is helpful to you.
Thank you for your question.

More Info
Long-term use of sennoside, a common over-the-counter laxative derived from senna leaves, can indeed raise concerns regarding side effects and dependency. Sennoside works by stimulating the lining of the bowel, which promotes bowel movements. While it can be effective for short-term relief of constipation, prolonged use can lead to several issues.
Side Effects of Long-Term Sennoside Use
1. Electrolyte Imbalance: Chronic use of sennoside can lead to electrolyte imbalances, particularly low potassium levels (hypokalemia). This can result in muscle weakness, cramping, and irregular heart rhythms.
2. Dependency: Over time, the bowel may become reliant on sennoside for stimulation, leading to a condition known as "laxative dependency." This means that the bowel may lose its natural ability to function properly without the aid of laxatives, making it difficult to have a bowel movement without them.
3. Bowel Dysfunction: Long-term use can also lead to a condition called "cathartic colon," where the colon becomes dilated and loses its ability to contract effectively. This can result in chronic constipation and may require medical intervention.
4. Gastrointestinal Issues: Some individuals may experience abdominal cramping, diarrhea, or nausea as side effects of sennoside, especially if taken in higher doses than recommended.
Alternatives for Constipation Relief
Given your situation, where traditional methods like dietary changes and increased hydration have not yielded results, it may be beneficial to explore alternative treatments:
1. Fiber Supplements: Products containing psyllium husk or methylcellulose can help increase stool bulk and promote regularity without the risks associated with stimulant laxatives.
2. Osmotic Laxatives: Medications like polyethylene glycol (Miralax) draw water into the bowel, softening the stool and making it easier to pass. They are generally considered safer for long-term use compared to stimulant laxatives.
3. Probiotics: These can help improve gut health and may aid in regulating bowel movements. Foods rich in probiotics, such as yogurt or fermented foods, can be beneficial.
4. Lifestyle Modifications: While you've mentioned trying various lifestyle changes, it might be worth revisiting these with a focus on regular exercise, a balanced diet rich in fiber, and adequate hydration. Sometimes, a structured routine can help.
5. Consultation with a Specialist: Since you have undergone extensive testing without finding a clear cause for your constipation, it may be helpful to consult a gastroenterologist or a dietitian specializing in digestive health. They can provide tailored advice and explore other potential underlying issues.
Regarding Powdered Rhubarb and Powdered Aloe
Both powdered rhubarb and powdered aloe are herbal remedies that have been used for constipation relief, but they also come with potential side effects:
- Powdered Rhubarb: This can act as a laxative, but long-term use may lead to similar issues as sennoside, including dependency and electrolyte imbalances. It may also cause abdominal cramping and diarrhea.
- Powdered Aloe: Aloe vera latex contains compounds that can stimulate bowel movements. However, it can also lead to cramping, diarrhea, and potential electrolyte disturbances. Long-term use is not recommended due to the risk of developing a dependency.
Conclusion
In summary, while sennoside can be effective for short-term relief of constipation, long-term use poses risks that should not be overlooked. It is essential to consider alternative treatments and consult with healthcare professionals to develop a comprehensive plan tailored to your needs. Regular monitoring of your condition and any side effects is crucial to ensure your long-term health and well-being.
