The skin is red and swollen?
Hello, I have some personal concerns.
Recently, I have developed about ten small bumps around my genital area, specifically near the testicles and around the anus.
They are itchy, especially in the morning, and scratching makes the itching worse.
I have previously experienced allergy-like symptoms on my skin, but seeing a doctor did not help.
Later, I discovered that the symptoms resembled those of scabies.
I went to the pharmacy and bought scabies treatment to apply at home.
The symptoms have improved, but now there are red, bump-like lesions around my genital area that resemble what I had when my skin was itchy.
I would like to ask the doctor if the scabies could have transferred to my genital area or if I might have some other condition, such as tinea.
I did not apply the scabies treatment thoroughly before.
I would appreciate your guidance on which specialist I should see.
Thank you for your time! If possible, I can email photos for the doctor to review.

Dr. Jiang Zheen reply Dermatology
You should see a dermatologist, but I believe your scrotal issue may be a consequence of scabies.
Local corticosteroid injections can be used for treatment and are quite effective.
Wishing you peace and may God bless you.

More Info
It sounds like you're experiencing a concerning issue with red bumps and itching in the genital area, which can be quite distressing. Based on your description, there are several potential causes for these symptoms, and I’ll provide some insights into what might be happening and what steps you can take.
Possible Causes of Red Bumps and Itching
1. Folliculitis: This is an inflammation of the hair follicles that can occur in the genital area. It can be caused by bacteria, fungi, or irritation from shaving or tight clothing. The bumps may appear red and can be itchy.
2. Contact Dermatitis: If you've had previous allergic reactions, it’s possible that you are experiencing contact dermatitis. This condition occurs when the skin reacts to an irritant or allergen, leading to red, itchy bumps.
3. Scabies: You mentioned that you had symptoms similar to scabies, which is caused by a mite that burrows into the skin. Scabies can cause intense itching and a rash, often in the genital area. If you suspect scabies, it’s important to treat it properly, as it can spread easily.
4. Fungal Infections: Conditions like jock itch (tinea cruris) can cause red, itchy rashes in the groin area. This is a fungal infection that thrives in warm, moist environments.
5. Sexually Transmitted Infections (STIs): Certain STIs can present with symptoms like bumps or sores in the genital area. It’s crucial to consider this possibility, especially if there has been any change in sexual partners or practices.
6. Pearly Penile Papules: If the bumps are small, dome-shaped, and located around the head of the penis, they may be pearly penile papules, which are harmless and common.
Recommended Actions
1. Avoid Scratching: While it may be tempting to scratch the itchy areas, doing so can worsen the irritation and lead to secondary infections.
2. Keep the Area Clean and Dry: Maintain good hygiene by washing the area gently with mild soap and water. After washing, ensure the area is thoroughly dried, as moisture can exacerbate fungal infections.
3. Over-the-Counter Treatments: If you suspect a fungal infection, antifungal creams available at pharmacies may help. For allergic reactions, hydrocortisone cream can reduce inflammation and itching.
4. Consult a Healthcare Provider: Given the persistence of your symptoms and the potential for various underlying causes, it’s advisable to see a healthcare provider. A dermatologist or a urologist would be appropriate specialists to consult. They can perform a physical examination, possibly take a sample for testing, and provide a definitive diagnosis and treatment plan.
5. Avoid Self-Diagnosis and Treatment: While it’s good that you’ve tried to address the issue with over-the-counter treatments, self-diagnosing can sometimes lead to incorrect treatments that may worsen the condition.
Conclusion
In summary, the red bumps and itching you’re experiencing could be due to a variety of causes, including folliculitis, contact dermatitis, scabies, fungal infections, or even STIs. It’s essential to avoid scratching, maintain hygiene, and consult a healthcare provider for an accurate diagnosis and appropriate treatment. Early intervention can help prevent complications and alleviate your symptoms effectively. If you have any further questions or need clarification, feel free to ask!
