Scrotal skin
I have many small, white, grain-like bumps on the skin of my testicles.
They are painless and itch-free, and they have been there for a long time.
What could these be?

Dr. Cai Bingkun reply Urology
Ming: Based on your complaint of having some white lumps on the skin of your scrotum, it seems to be epidermoid cysts, which contain keratinized material, similar to facial acne.
Alternatively, it could be unexplained skin calcification (containing calcium phosphate).
Neither condition should pose any health issues.
However, if you are very concerned, you may seek assistance from a dermatologist.

The presence of white bumps on the scrotum can be concerning, but it is important to understand that there are several potential causes for these bumps, many of which are benign. Given your description of the bumps as small, white, and painless, it is likely that they could be one of the following:
1. Pearly Penile Papules: These are small, dome-shaped bumps that typically appear around the corona of the glans penis and can sometimes extend to the scrotum. They are a normal anatomical variation and are not associated with any disease or health risk. They are harmless and do not require treatment unless they cause psychological distress.
2. Sebaceous Cysts: These are small, non-painful lumps that can occur anywhere on the body, including the scrotum. They are caused by blocked sebaceous glands and can appear white or yellowish. These cysts are generally harmless but can become infected, in which case they may require drainage or removal.
3. Folliculitis: This condition occurs when hair follicles become inflamed, often due to infection or irritation. It can lead to small, red, or white bumps that may be itchy or painful, but if you are not experiencing any discomfort, this may not be the case for you.
4. Fordyce Spots: These are small, painless, pale bumps that are actually enlarged sebaceous glands. They are completely normal and do not require treatment.
5. Genital Warts: Caused by the human papillomavirus (HPV), these can appear as small, flesh-colored or white bumps. They may be painless but can sometimes itch or cause discomfort. If you suspect this might be the case, it is important to consult a healthcare provider for evaluation and potential treatment.
6. Other Skin Conditions: Conditions such as eczema or psoriasis can also cause changes in the skin, including bumps or patches. These conditions may require specific treatments based on their severity and symptoms.
Given that you have described these bumps as being present for a long time and not causing any pain or itching, it is likely that they are benign. However, it is always best to consult with a healthcare provider for an accurate diagnosis, especially if there are any changes in the appearance of the bumps, or if you develop any new symptoms.
Recommendations:
- Observation: If the bumps are not causing any discomfort and have not changed in size or appearance, you may choose to monitor them.
- Hygiene: Maintain good hygiene in the genital area to prevent any potential infections.
- Avoid Irritation: Wear loose-fitting underwear and avoid harsh soaps or products that may irritate the skin.
- Consult a Healthcare Provider: If you have any concerns or if the bumps change in appearance, size, or if you develop any new symptoms, it is advisable to see a healthcare provider. They may perform a physical examination and, if necessary, conduct tests to determine the exact nature of the bumps.
In conclusion, while the presence of white bumps on the scrotum can be alarming, many causes are benign and require no treatment. However, professional evaluation is always recommended to rule out any serious conditions and to provide peace of mind.
