Weak urine flow
Hello: My urine stream seems weak (I don't feel the urge to control it), and shortly after drinking water, I feel the need to urinate.
Additionally, my ejaculation lacks force (I don't feel the sensation of ejaculation, just a flow).
I am 37 years old, and these symptoms have been present for 3 to 4 months.
What could this possibly indicate? Thank you!
Xiao Ming, 30~39 year old female. Ask Date: 2004/08/15
Dr. Pan Zhenqin reply Urology
A 37-year-old man should experience a smooth and free flow during urination or ejaculation.
If you are experiencing the issues you mentioned, it is advisable to undergo further examination for potential urethral stricture or other problems.
I recommend visiting a urology clinic.
Wishing you good health from the Tainan Hospital of the Department of Health!

Understanding weak urination can be concerning, especially when it persists for several months. The symptoms you describe—weak urine flow, a lack of control over urination, and diminished sensation during ejaculation—could be indicative of several underlying medical conditions.
Firstly, it’s important to consider the possibility of a urinary tract infection (UTI) or bladder dysfunction. UTIs can cause changes in urination patterns, including urgency and weak flow. However, given that your symptoms have lasted for several months, a UTI may be less likely unless it is recurrent.
Another potential cause could be benign prostatic hyperplasia (BPH), which is an enlargement of the prostate gland that can occur in men as they age. BPH can lead to urinary symptoms such as weak stream, difficulty starting urination, and a feeling of incomplete bladder emptying. At 37, while it is somewhat early for BPH, it is not impossible, especially if there is a family history of prostate issues.
Neurological conditions could also be a factor. Conditions affecting the nerves that control the bladder and pelvic floor muscles can lead to weak urination. This includes conditions such as multiple sclerosis or spinal cord injuries, which can disrupt the signals between the brain and the bladder.
Additionally, pelvic floor dysfunction can contribute to weak urination. The pelvic floor muscles support the bladder and help control urination. If these muscles are weak or not functioning properly, it can lead to difficulties with urination and ejaculation.
Hormonal imbalances, particularly involving testosterone, can also affect urinary and sexual function. Low testosterone levels can lead to decreased libido and changes in urinary function.
Given the complexity of your symptoms, it is crucial to consult a healthcare professional for a thorough evaluation. A urologist would be particularly well-suited to assess your symptoms. They may perform a physical examination, including a digital rectal exam to assess the prostate, and may recommend tests such as a urinalysis, ultrasound, or urodynamic studies to evaluate bladder function.
In terms of immediate steps you can take, ensure you are staying well-hydrated, as dehydration can exacerbate urinary issues. Avoiding irritants such as caffeine, alcohol, and spicy foods may also help reduce symptoms.
In conclusion, while weak urination can stem from various causes, it is essential to seek medical advice to determine the underlying issue and appropriate treatment. Early intervention can often lead to better outcomes and help alleviate your symptoms. Please do not hesitate to reach out to a healthcare provider to discuss your concerns in detail.
