Failed double eyelid surgery?

Dear Doctor: I underwent double eyelid surgery and canthoplasty with fat removal five months ago.
However, the surgeon made the incisions too large, resulting in my double eyelids appearing thick and swollen.
Additionally, there is a noticeable asymmetry between the two sides.
What should I do to address this issue?

NINI, 20~29 year old female. Ask Date: 2004/08/16

Dr. Luo Qihua reply Plastic Surgery

After five months, most of the swelling should have subsided.
If there is still a feeling of thickness or swelling, it may be due to a wider incision or an excess of fat.
A thorough evaluation is necessary to determine the cause.

Dealing with uneven and thick double eyelids after surgery can be a frustrating experience, especially when the results do not meet your expectations. It is important to understand that post-operative healing can take time, and there are several factors that can contribute to the appearance of your eyelids. Here are some steps and considerations to help you address your concerns:
1. Consult Your Surgeon: The first step is to schedule a follow-up appointment with the surgeon who performed your double eyelid surgery. They will be able to assess your eyelids and determine if the thickness and unevenness are due to swelling, scarring, or surgical technique. It’s essential to communicate your concerns clearly, as they can provide tailored advice based on your specific situation.

2. Understanding Healing Time: After eyelid surgery, it is common for swelling to persist for several weeks or even months. The final results may not be visible until the swelling has completely subsided. In many cases, the eyelids will continue to soften and flatten as the healing process progresses. If you are five months post-surgery, you may still see changes in the appearance of your eyelids, but it is crucial to have realistic expectations about the timeline.

3. Managing Swelling: If your eyelids are still swollen, there are several methods to help reduce this. Cold compresses can be applied to the eyelids to alleviate swelling. Additionally, keeping your head elevated while sleeping can help minimize fluid retention. If your surgeon has prescribed any medications or topical treatments, be sure to follow their instructions carefully.

4. Consider Non-Surgical Options: If the unevenness and thickness do not improve with time, you may want to explore non-surgical options such as fillers or laser treatments to help balance the appearance of your eyelids. These options can sometimes provide a temporary solution while you consider further surgical intervention.

5. Surgical Revision: If after a reasonable period of healing (usually at least six months), you are still unhappy with the results, you may want to discuss the possibility of a revision surgery with your surgeon. Revision surgery can help correct issues such as uneven eyelid folds or excessive thickness. However, it is essential to ensure that you are fully healed from the initial surgery before considering this option.

6. Follow-Up Care: Regular follow-up appointments with your surgeon are crucial for monitoring your healing process. They can provide guidance on what to expect and when to seek further intervention if necessary. If you notice any signs of complications, such as increased redness, pain, or discharge, it is important to contact your surgeon immediately.

7. Emotional Support: Dealing with dissatisfaction after cosmetic surgery can take an emotional toll. It may be helpful to talk to friends, family, or a counselor about your feelings. Support groups for individuals who have undergone similar procedures can also provide a sense of community and understanding.

In summary, while it can be disheartening to experience uneven and thick double eyelids after surgery, there are steps you can take to address these concerns. Consulting with your surgeon is the most important first step, as they can provide personalized advice and treatment options. Remember that healing takes time, and it is essential to be patient with the process. If necessary, explore both non-surgical and surgical options for correction, and seek emotional support to help navigate this challenging experience.
