Facial sweating is profuse, like rain!
Hello: I have been sweating a lot since I was young, and I tend to flush when I get nervous, but it has never been severe enough to be unbearable.
However, in recent years, I have noticed that I sweat a lot just from walking a few steps.
In addition to excessive sweating on my face, chest, and back, I have also experienced sweating on the soles of my feet (which I didn't have as a child, but it has recently returned).
In the summer, my hands and feet feel hot, and in the winter, my back gets warm (unless there is a cold front, I still sweat even in short sleeves).
As for common hand sweating, it occurs rarely (I have experienced it, but it was mild).
Could you please advise which specialty I should see for my almost systemic hyperhidrosis? Which hospital in the Kaohsiung-Pingtung area is more specialized in this? Thank you.

Hello: Generally speaking, palm sweating is a secondary condition; however, your situation seems to indicate generalized hyperhidrosis, which may suggest an issue with your endocrine system.
Additionally, you should consider whether you experience irritability, have a large appetite while maintaining a low body weight, or have high blood pressure.
If so, this could indicate hyperthyroidism.
It would be advisable to consult a physician specializing in metabolism for a thorough evaluation and treatment.
Sincerely, Dr.

More Info
Excessive sweating, also known as hyperhidrosis, can be a challenging condition that affects many individuals. It can manifest in various forms, including generalized sweating or localized sweating in specific areas such as the palms, feet, or underarms. Given your description of experiencing excessive sweating in various situations, including physical activity and changes in temperature, it is essential to understand when to seek medical help and what steps to take next.
When to Seek Medical Help
1. Severity of Symptoms: If your sweating is interfering with your daily activities, such as work, social interactions, or personal hygiene, it is advisable to consult a healthcare professional. Excessive sweating can lead to emotional distress and social anxiety, which can further impact your quality of life.
2. Changes in Sweating Patterns: If you notice a sudden increase in sweating or changes in the areas where you sweat, it could indicate an underlying medical condition. Conditions such as hyperthyroidism, diabetes, or infections can cause excessive sweating, and it is crucial to rule these out.
3. Associated Symptoms: If your sweating is accompanied by other symptoms such as weight loss, fever, or fatigue, it is essential to seek medical attention promptly. These could be signs of a more serious health issue that requires immediate evaluation.
4. Family History: If there is a family history of hyperhidrosis or other related conditions, it may be beneficial to consult a specialist to understand your risk and explore treatment options.
Which Specialist to See
In your case, the first step would be to consult a dermatologist. Dermatologists specialize in skin conditions and can provide insights into the causes of excessive sweating. They may perform a physical examination, review your medical history, and possibly conduct tests to determine the underlying cause of your hyperhidrosis.
If the dermatologist finds that your condition is more complex or related to other systemic issues, they may refer you to an endocrinologist. Endocrinologists specialize in hormonal and metabolic disorders and can evaluate conditions like hyperthyroidism or diabetes that may contribute to excessive sweating.

Treatment Options

- Topical Antiperspirants: Prescription-strength antiperspirants containing aluminum chloride can be effective for localized sweating.
- Botox Injections: Botulinum toxin injections can temporarily block the nerves that cause sweating and are particularly effective for underarm sweating.
- Medications: Oral medications that reduce sweating, such as anticholinergics, may be prescribed.
- Iontophoresis: This treatment involves using a device that passes a mild electrical current through water and into the skin's surface, which can help reduce sweating in the hands and feet.
- Surgery: In severe cases, surgical options such as sympathectomy may be considered, where nerves responsible for sweating are cut.
In conclusion, if you are experiencing excessive sweating that affects your daily life, it is essential to seek medical help. Start with a dermatologist who can evaluate your condition and guide you toward appropriate treatment options. Remember, you are not alone in this, and effective treatments are available to help manage your symptoms.
