Stretch marks

1.
Is using pregnancy cream effective in preventing stretch marks, or do lotions have the same effect? The price difference between the two is significant!
2.
Is itching on the belly related to "toxins" from the pregnancy? I've heard from elders that drinking bitter melon stew with pork ribs can help.
Is this effective?
3.
If I use anti-itch cream for belly itching, are there restrictions on using those that contain steroids? (It seems that dermatological medications often contain steroids; will this be absorbed by the skin?)

Xiaomin, 20~29 year old female. Ask Date: 2001/03/20

Dr. Zhan Deqin reply Obstetrics and Gynecology

1.
According to most reports, it has little to no effect.
2.
Abdominal itching is caused by cholestasis related to pregnancy, and drinking bitter melon is certainly ineffective.
3.
Most anti-itch medications contain steroids, although there are some that do not (like hydrocortisone cream).
A small amount of steroids will not affect pregnancy, as long as they are not used in large quantities over a long period.

Understanding stretch marks, also known as striae, is essential for many expecting mothers and individuals experiencing rapid weight changes. Stretch marks occur when the skin is stretched rapidly due to factors such as pregnancy, puberty, or significant weight gain. They appear as streaks on the skin, often in colors ranging from red to purple and eventually fading to a lighter shade. Here, we will address the questions regarding prevention, remedies, and common myths associated with stretch marks.

1. Effectiveness of Stretch Mark Creams vs. Lotions:
When it comes to preventing stretch marks, many women wonder if specialized pregnancy creams are more effective than regular lotions. While some creams are marketed specifically for stretch mark prevention, the primary goal of these products is to keep the skin moisturized. Regular moisturizers or lotions can also provide similar benefits by maintaining skin elasticity and hydration. The key ingredients often found in pregnancy creams include cocoa butter, shea butter, and hyaluronic acid, which can help improve skin elasticity. However, the price difference can be significant, and many women find that a good quality lotion can be just as effective. Ultimately, the best approach is to keep the skin well-hydrated, regardless of the product used.

2. Itchy Belly During Pregnancy:
The sensation of an itchy belly during pregnancy is common and can be attributed to several factors, including skin stretching and hormonal changes. The term "胎毒" (tai du), or "fetal toxins," is a traditional belief in some cultures, suggesting that certain foods or remedies can alleviate discomfort during pregnancy. Drinking bitter melon soup with pork ribs is a traditional remedy believed to help with various pregnancy-related issues, but scientific evidence supporting its effectiveness for itchy skin is limited. Instead, it is advisable to focus on keeping the skin moisturized and avoiding irritants. If the itching becomes severe or is accompanied by a rash, it is essential to consult a healthcare provider, as it could indicate a condition like pruritic urticarial papules and plaques of pregnancy (PUPPP) or cholestasis of pregnancy.

3. Use of Anti-Itch Creams:
When considering anti-itch creams, it is crucial to be cautious about the ingredients, especially regarding corticosteroids. While topical steroids can effectively relieve itching and inflammation, prolonged use during pregnancy should be approached with caution. Low-potency corticosteroids may be prescribed by a healthcare provider if deemed necessary, but it is essential to follow their guidance. The concern about systemic absorption is valid; however, when used as directed and in small areas, the risk is generally low. Non-steroidal options, such as calamine lotion or creams containing menthol or pramoxine, can also provide relief without the risks associated with steroids.

In summary, preventing stretch marks involves maintaining skin hydration, and both specialized creams and regular lotions can be effective. Itchy skin during pregnancy is common and can often be managed with proper skincare. While some traditional remedies may provide comfort, consulting a healthcare professional is always recommended for persistent symptoms. Lastly, when using anti-itch creams, it is essential to be mindful of the ingredients and consult with a healthcare provider to ensure safety during pregnancy.
