Frequent urination
Hello Dr.
Chen,
I would like to ask if varicocele can cause frequent urination, especially noticeable at night when lying down.
Additionally, I have recently experienced pain in the right thigh near the anus; it occurs when bending over, straining the lower abdomen, or lifting the right leg.
Could this be related to a prostate issue? I have had a urinalysis, and the results were normal.
If fertility is not a concern, is surgery still necessary for varicocele?
kk, 40~49 year old female. Ask Date: 2004/08/29
Dr. Chen Xiuyi reply Urology
Hello: Varicocele is rarely associated with frequent urination.
The causes of frequent urination are often related to issues in the lower urinary tract, including the bladder, prostate, urethra, and spinal cord, which can all contribute to this symptom.
It is advisable to consult a urologist for a differential diagnosis.
The main reasons for surgical intervention in cases of varicocele include testicular pain and infertility.
Additionally, a small number of patients may develop varicocele later in life, and it is important to rule out the possibility of associated renal tumors.
Sincerely, Dr.
Chen Hsiu-Yi, Urology.

More Info
Varicocele, which is an enlargement of the veins within the scrotum, is commonly associated with male infertility but can also lead to various symptoms, including pelvic pain. However, the relationship between varicocele and symptoms such as frequent urination and pelvic pain is not straightforward.
Understanding Varicocele
Varicocele occurs when the valves in the veins that carry blood from the testicles to the heart fail, leading to blood pooling in the veins. This condition is often likened to varicose veins in the legs. While varicocele is primarily a concern for fertility, it can also cause discomfort or pain in the scrotal area, which may radiate to the lower abdomen or groin.
Symptoms of Varicocele
Patients with varicocele may experience:
- Dull, aching pain in the scrotum, which may worsen after prolonged standing or physical activity.
- A feeling of heaviness in the affected testicle.
- Visible or palpable enlarged veins in the scrotum, often described as a "bag of worms."
Frequent Urination and Pelvic Pain
Regarding your specific questions about frequent urination and pelvic pain:
1. Frequent Urination: Varicocele is not typically associated with urinary symptoms such as frequent urination. However, if the varicocele is causing significant discomfort or pain, it may lead to changes in urinary habits due to discomfort during urination or a psychological response to pain. Other conditions, such as prostate issues or urinary tract infections, are more commonly associated with frequent urination, especially at night (nocturia).
2. Pelvic Pain: The pain you describe in your right thigh and lower abdomen, especially when bending or lifting your leg, could potentially be related to the varicocele, but it may also indicate other issues, such as a hernia, muscle strain, or even referred pain from the prostate. Given that your urinalysis results are normal, it suggests that a urinary tract infection or other acute urinary issues are less likely.
Should You Consider Surgery?
In terms of surgical intervention for varicocele, the decision often depends on several factors:
- Symptoms: If the varicocele is causing significant pain or discomfort that affects your quality of life, surgical intervention may be warranted.
- Fertility Concerns: If you are considering fathering children in the future, surgery may be recommended to improve sperm quality and fertility potential.
- Severity of the Condition: If the varicocele is classified as severe and is causing significant symptoms, surgical options such as varicocelectomy may be considered.
If fertility is not a concern for you, and the varicocele is not causing debilitating pain, many healthcare providers may recommend a conservative approach, including monitoring the condition and managing symptoms with pain relief methods.
Conclusion
In summary, while varicocele can cause discomfort and pain, it is not typically associated with frequent urination. Your symptoms warrant further evaluation to rule out other potential causes, particularly if they persist or worsen. Consulting with a urologist would be beneficial for a comprehensive assessment and to discuss the best management options tailored to your specific situation.
