Penile Discomfort: Symptoms and Possible Causes - Urology

Excuse me, doctor... I have a few minor questions I would like to ask... Thank you?

Doctor, about a week ago, I started feeling discomfort in my penis.
It feels sore and painful during erections.
I have a curvature, but I have never had any issues before.
Initially, I noticed that it felt warmer than usual, and urination also felt warm but not painful.
A few days ago, I went to the hospital for a urine test, and the doctor said everything was normal, with urine temperature also being warm.
The doctor suggested it might be a mild urethritis and prescribed anti-inflammatory medication.
After taking it for two days, I feel somewhat better, but I still experience soreness during erections.
The painful area seems to be on the soft part underneath the penis (not muscle), and sometimes pressing on the sides also feels sore.
Additionally, I have a slight urge to urinate after an erection, and the base feels sore as well.
Could you please help me understand what these symptoms might indicate? If you need to know more about my condition to explain, please let me know, and I will do my best to provide the information.
Thank you, doctor.

Dr. Lin Hexing reply Urology

Hello Ah Heng, the pain you describe during an erection is located in the soft area beneath the penis, which is the corpus spongiosum region.
Your symptoms are indeed more indicative of urethritis.
I'm curious if you usually drink enough water, specifically more than 2000 cc? I recommend that you increase your water intake.
Currently, do you experience any difficulties when urinating? If not, it is unlikely to be a serious issue.
You may consider repeating a urinalysis and continuing with symptomatic treatment.

More Info

Penile discomfort can be a concerning symptom, and it’s important to understand the potential causes and implications of your symptoms. Based on your description, it seems you are experiencing discomfort during erections, a sensation of warmth, and some pain associated with urination. These symptoms can arise from various conditions, and I will outline some possible causes and considerations.

Possible Causes of Penile Discomfort
1. Urethritis: This is an inflammation of the urethra, which can be caused by infections (bacterial or viral) or irritants. Symptoms often include pain during urination, discomfort in the penis, and sometimes discharge. Since you mentioned that your doctor suspected mild urethritis and prescribed anti-inflammatory medication, this could be a likely cause of your discomfort.

2. Peyronie's Disease: Given that you mentioned having a curvature in your penis, it’s worth considering Peyronie's disease, which is characterized by the development of fibrous scar tissue inside the penis, causing curved, painful erections. This condition can lead to discomfort during erections and may require further evaluation by a urologist.

3. Penile Trauma or Injury: If you have experienced any recent trauma to the genital area, even if minor, it could lead to discomfort. This could include activities such as vigorous sexual activity or injury during sports.

4. Infection: Infections such as sexually transmitted infections (STIs) can cause discomfort, pain during erections, and other symptoms. If there is any possibility of exposure to STIs, it would be prudent to get tested.

5. Prostatitis: Inflammation of the prostate gland can cause pelvic pain, discomfort during erections, and urinary symptoms. This condition can be acute or chronic and may require specific treatment.

6. Nerve Irritation or Damage: Sometimes, nerve issues can lead to discomfort in the genital area. This could be due to compression or irritation of nerves in the pelvic region.

Recommendations
1. Follow-Up with Your Doctor: Since you have already seen a doctor and received medication, it’s important to follow up if your symptoms persist or worsen. If the discomfort during erections continues, a urologist may need to evaluate you further.

2. Monitor Symptoms: Keep track of any changes in your symptoms, including the intensity of pain, any new symptoms, or changes in urination. This information can be helpful for your healthcare provider.

3. Avoid Irritants: Until you have a clearer diagnosis, it may be wise to avoid any potential irritants, such as harsh soaps or lotions in the genital area, and to practice safe sex to prevent any potential infections.

4. Consider Lifestyle Factors: Factors such as stress, anxiety, and lifestyle choices can impact sexual health. If you feel that stress or anxiety may be contributing to your symptoms, consider discussing this with your healthcare provider.

5. Stay Hydrated and Maintain Hygiene: Ensure you are drinking enough water and maintaining good hygiene, as this can help prevent urinary tract infections and other issues.

Conclusion
Penile discomfort can stem from various causes, and while mild urethritis is a possibility, it’s essential to consider other factors, especially given your history of penile curvature. Continued communication with your healthcare provider is crucial for proper diagnosis and treatment. If symptoms persist or worsen, do not hesitate to seek further medical attention. Your health and comfort are paramount, and addressing these symptoms early can lead to better outcomes.
