Abdomen

Hello doctor: Recently, I have been experiencing discomfort in the upper middle part of my abdomen, and there is a sharp pain under the left side of my rib cage.
I also have hepatitis B, and I'm unsure what the issue might be.
Thank you.

Hello, the pain in the upper abdomen and left upper abdomen is more likely related to gastric issues, such as a gastric ulcer.
Do you experience increased pain when hungry or a feeling of bloating after meals? Hepatitis B does carry a higher risk of liver cancer, but the liver and gallbladder are located in the right upper abdomen, so it is less likely to be related to liver or gallbladder issues.
I still recommend that you consult a gastroenterologist for further evaluation.
Sincerely, Dr.
Chen Zhi-Ping, Internal Medicine.
Thank you!

Upper abdominal discomfort and pain can be concerning, especially for individuals with chronic Hepatitis B. Your symptoms, which include discomfort in the upper abdomen and sharp pain under the left rib, warrant careful evaluation. Given your history as a Hepatitis B carrier, it is essential to consider various potential causes for your symptoms.

Firstly, the upper abdomen is home to several vital organs, including the liver, stomach, spleen, and pancreas. In patients with Hepatitis B, liver-related issues are a primary concern. While Hepatitis B itself may not always cause pain, it can lead to liver inflammation, fibrosis, or even cirrhosis over time. These conditions can manifest as discomfort or pain in the upper right quadrant of the abdomen, where the liver is located. However, your left-sided pain may suggest other underlying issues.

The left upper quadrant pain you are experiencing could be related to several factors. One possibility is gastric issues, such as gastritis or peptic ulcers, which can cause referred pain to the left side of the abdomen. Given your history of duodenal ulcers, it is crucial to consider that you may be experiencing a recurrence or a new gastric issue. Symptoms of gastric ulcers often include pain that worsens when the stomach is empty or after eating.

Another consideration is splenic issues. The spleen is located in the left upper quadrant and can become enlarged or inflamed due to various conditions, including infections or hematological disorders. An enlarged spleen can cause discomfort or pain in the left upper abdomen.

Additionally, it is essential to rule out any biliary or pancreatic issues. Conditions such as gallstones or pancreatitis can also present with upper abdominal pain, although they typically cause pain that radiates to the back or is associated with nausea and vomiting.

Given your Hepatitis B status and the presence of abdominal pain, it is advisable to seek further evaluation. A comprehensive assessment by a gastroenterologist or hepatologist is recommended. They may suggest imaging studies, such as an abdominal ultrasound, to evaluate the liver, gallbladder, spleen, and other abdominal organs. Blood tests to assess liver function and check for any signs of liver inflammation or damage would also be beneficial.

In terms of management, lifestyle modifications can play a significant role in maintaining liver health. It is crucial to avoid alcohol, maintain a balanced diet, and ensure adequate hydration. Regular follow-ups with your healthcare provider to monitor liver function and any potential progression of liver disease are essential.

In summary, while your symptoms could be related to your Hepatitis B status, they may also indicate other gastrointestinal or splenic issues. A thorough evaluation is necessary to determine the underlying cause of your discomfort and to develop an appropriate management plan. Do not hesitate to reach out to your healthcare provider for further assessment and guidance.
