There are suddenly white flakes on the glans?
Hello Doctor: A few months ago, I suddenly noticed white flakes on my glans, which could be washed off.
At first, I didn't think much of it, but after a few days, my foreskin became red and swollen, and my urination became irregular and painful.
I researched online and thought it might be glans inflammation, so I went to the pharmacy and bought an ointment to apply.
During the treatment with the ointment, it seemed to improve, but unfortunately, within a week of stopping the medication, it relapsed.
I tried different ointments, but the situation remained the same (I have tried four different ones).
I would like to ask the doctor if, besides seeking treatment at a clinic, I can self-medicate with over-the-counter medications.
If I do self-medicate, what should I pay attention to? Should I use oral medication and ointment simultaneously? Thank you for your response, Doctor.

Dr. Liu Jianting reply Urology
It might be a fungal infection! Please consult a urologist or dermatologist, and avoid buying medication on your own.

More Info
The symptoms you are describing, including the presence of white flakes on the glans (the head of the penis), redness, swelling, and irregular urination, could be indicative of a few different conditions. The most common causes of these symptoms include balanitis (inflammation of the glans), yeast infections, or other dermatological conditions such as psoriasis or eczema.
Understanding the Condition
1. Balanitis: This is an inflammation of the glans, often caused by poor hygiene, irritants, or infections. Symptoms typically include redness, swelling, and sometimes discharge or flakes. It can be exacerbated by the use of soaps or other products that irritate the sensitive skin of the genital area.
2. Yeast Infection: A common cause of white flakes or patches, particularly if accompanied by itching or irritation. Yeast infections can occur in men, especially if they have diabetes or are on antibiotics.
3. Psoriasis or Eczema: These skin conditions can also affect the genital area, leading to flaking and inflammation. They are often chronic and may require ongoing management.
Treatment Options
Given that you have tried multiple topical treatments without success, it may be time to consider a more comprehensive approach:
1. Consult a Dermatologist or Urologist: While over-the-counter treatments can be helpful, a healthcare professional can provide a definitive diagnosis and tailored treatment plan. They may perform tests to determine the exact cause of your symptoms.
2. Topical Treatments: If a fungal infection is suspected, antifungal creams (like clotrimazole or miconazole) may be effective. For inflammatory conditions, topical steroids (like hydrocortisone) may help reduce inflammation.
3. Oral Medications: If the condition is severe or persistent, your doctor may prescribe oral antifungal medications or other systemic treatments.
4. Hygiene Practices: Maintaining good hygiene is crucial. Gently wash the area with mild soap and water, and ensure it is dry before dressing. Avoid irritants such as scented soaps or lotions.
5. Avoid Self-Medicating: While it may be tempting to continue trying different over-the-counter creams, this can sometimes worsen the condition or mask the underlying issue. It’s essential to have a clear diagnosis before starting any new treatment.
Self-Care and Prevention
- Avoid Irritants: Be cautious with soaps, lotions, and other products that may irritate the genital area.
- Wear Breathable Fabrics: Cotton underwear can help keep the area dry and reduce irritation.
- Diet and Health: If you have underlying health issues like diabetes, managing those conditions can help prevent infections.
- Follow-Up: If symptoms persist despite treatment, follow up with your healthcare provider for further evaluation.
Conclusion
In summary, while self-treatment is possible, it is often not the most effective approach for persistent or recurrent symptoms. A healthcare professional can provide a proper diagnosis and treatment plan tailored to your specific condition. If you suspect a yeast infection or another dermatological issue, seeking medical advice is the best course of action to ensure effective treatment and prevent recurrence.
