The right testicle feels abnormal?
Hello, doctor.
Today I noticed something unusual about my left testicle.
The structure connecting the testicle seems to have become thicker and larger.
It feels like there are lumps or many more cords when I touch it, extending from the testicle down to the base of the penis.
I'm not sure how to describe it, but it doesn't hurt or feel uncomfortable; I just happened to notice it while touching.
The right side feels normal, and I can't feel anything unusual there, but the left side is very noticeable, almost like a "mass." Could this be related to some kind of vein issue? Do I need to consider surgery? I'm quite worried since I just discovered this in the evening.
This morning, I noticed I had ejaculated; I wonder if that is related? Thank you for your response.

Mr.
Huang: Hello! Your condition sounds like it could be due to several possibilities: 1.
Epididymitis 2.
Hernia 3.
Seminal vesicle cyst, or other scrotal masses.
It is also possible that it is simply a congenital variation in the shape of the epididymis (the structure connecting to the testis).
A detailed diagnosis can only be made after an examination at the outpatient clinic.

It sounds like you are experiencing some concerning changes in your left testicle, and it's understandable to feel anxious about it. Based on your description, it seems that you have noticed an unusual texture or enlargement in the area connecting the testicle to the body, which you described as feeling "grainy" or having more "cord-like" structures.
First and foremost, it’s important to recognize that changes in the testicles can be caused by a variety of conditions, some of which may require medical attention. The structures you are feeling could potentially be related to the spermatic cord or the epididymis, which is the coiled tube that stores and matures sperm. Here are a few possibilities to consider:
1. Epididymal Cysts or Spermatocele: These are fluid-filled sacs that can develop in the epididymis. They are usually benign and may not cause any pain or discomfort. They can sometimes feel like a lump or swelling.
2. Varicocele: This is a condition characterized by enlarged veins within the scrotum, similar to varicose veins in the legs. A varicocele can cause a feeling of heaviness or a "bag of worms" sensation in the scrotum, and it is more commonly found on the left side.
3. Testicular Torsion: This is a medical emergency where the spermatic cord becomes twisted, cutting off blood supply to the testicle. This condition is usually associated with severe pain, but if you are not experiencing pain, it is less likely to be the cause.
4. Inguinal Hernia: This occurs when tissue, such as part of the intestine, protrudes through a weak spot in the abdominal muscles. It can sometimes be felt in the scrotum and may cause a bulging sensation.
5. Testicular Tumors: While less common, any new lump or change in the testicle should be evaluated to rule out the possibility of testicular cancer. This is particularly important if you notice any changes in size, shape, or texture.
Given that you mentioned the left testicle feels different but is not painful, it is still crucial to have it evaluated by a healthcare professional. A physical examination by a urologist is recommended, as they can provide a thorough assessment and may suggest an ultrasound to visualize the structures within the scrotum.
As for your concern about the recent ejaculation, it is unlikely to be directly related to the changes you are feeling in your testicle. However, any significant changes in your body should be discussed with a doctor, especially if they are new or concerning to you.
In summary, while there are several benign explanations for the changes you are noticing, it is essential to seek medical advice to ensure that any potential issues are addressed promptly. Early diagnosis and treatment can make a significant difference in outcomes, especially if there is an underlying condition that requires intervention. Please do not hesitate to make an appointment with a urologist to discuss your symptoms and get the appropriate care.
