Irregular menstruation

Hello, I would like to ask about my irregular menstrual cycles over the past few years.
Sometimes my period comes late, sometimes early, and sometimes it doesn't come at all.
I have been intermittently taking medication to regulate my cycle, but after a few months of treatment, my periods become irregular again.
I am wondering if this could be related to gynecological tumors or something else, as I can't understand why my cycles keep becoming irregular after a short period of regulation.
Additionally, I have noticed that the flow is significantly less than before.
Although I have been taking medication to regulate my cycle, the flow is still not much.
I have been on this medication for the past two to three months, and while I have had regular periods during this time with a slightly improved flow, I am still very curious about why my cycles keep becoming irregular.
Should I undergo any further examinations, and what should I do?

Yatou, 30~39 year old female. Ask Date: 2004/11/10

Dr. Wu Zongcheng reply Obstetrics and Gynecology

Dr.
Wu Tsung-Cheng, the Director of the Department of Obstetrics and Gynecology at Tainan Hospital, Executive Yuan Health Department, responded:
Dear young lady,
The menstrual cycle is regulated by ovarian hormones.
You need to have a blood test to check your hormone levels in order to address the underlying issue of irregular menstruation.
Generally, what is referred to as menstrual regulation is merely symptomatic treatment (addressing the symptoms), which may help to establish a menstrual cycle temporarily.
However, after some time, the cycle may become irregular again.
I recommend the following: 1.
Have a blood test to check hormone levels.
2.
Undergo an ultrasound examination to check for any tumors.

Wishing you good health and all the best from Tainan Hospital!

Irregular menstruation can be a source of concern for many women, and it is essential to understand the potential causes, implications, and next steps for management. Irregular menstrual cycles can manifest as variations in cycle length, missed periods, or changes in menstrual flow, and they can be influenced by a variety of factors.

One of the most common causes of irregular menstruation is hormonal imbalance. Hormones such as estrogen and progesterone play crucial roles in regulating the menstrual cycle. Factors that can disrupt these hormones include stress, significant weight changes (either gain or loss), excessive exercise, and underlying medical conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders. PCOS, in particular, is a common endocrine disorder that can lead to irregular periods, weight gain, and other metabolic issues.

You mentioned that you have been taking medication to regulate your menstrual cycle, which indicates that you are already seeking treatment. However, the fact that your cycles return to irregularity after stopping the medication suggests that there may be an underlying issue that needs to be addressed. It is not uncommon for women to experience fluctuations in their menstrual cycles, especially during transitional phases of life, such as perimenopause, which can begin in the late 30s or early 40s.

Another concern you raised is the possibility of gynecological tumors. While irregular menstruation can be associated with conditions such as fibroids or endometrial hyperplasia, it is essential to note that these conditions often present with additional symptoms, such as pelvic pain, heavy bleeding, or unusual discharge. If you have not experienced these symptoms, it may be less likely that a tumor is the cause of your irregular cycles. However, it is always prudent to discuss any concerns with your healthcare provider.

Given your situation, it would be advisable to consider further evaluation. A thorough medical history, physical examination, and possibly imaging studies (such as an ultrasound) can help identify any structural abnormalities in the reproductive system. Additionally, blood tests to assess hormone levels, thyroid function, and other relevant markers can provide valuable insights into your menstrual irregularities.

If you are experiencing a significant change in menstrual flow, such as a decrease in volume, this should also be discussed with your healthcare provider. While some women may naturally have lighter periods, a sudden change can warrant further investigation to rule out any underlying conditions.

In summary, irregular menstruation can stem from various causes, including hormonal imbalances, lifestyle factors, and potential medical conditions. Since you have already been on medication to regulate your cycles, it may be beneficial to have a comprehensive evaluation to identify any underlying issues. Open communication with your healthcare provider is crucial, as they can guide you through the necessary tests and help develop a tailored treatment plan that addresses your specific needs. Remember, your health is paramount, and taking proactive steps towards understanding and managing your menstrual health is essential.
