Herpes?
Hello, doctor~~~ My boyfriend has developed recurrent herpes near the glans, which has led to ulcers.
Aside from sexual activity, are there any other causes of herpes infection?
Yan, 20~29 year old female. Ask Date: 2004/11/16
Dr. Xu Weikai reply Urology
Hello: Herpes is classified as a viral infection, and infection occurs only upon contact with the pathogen.
It is not solely transmitted through sexual intercourse; general daily contact and respiratory droplets can also lead to transmission.
In Taiwan, the prevalence of the herpes virus is approximately over 85%, which means that almost everyone has been infected.
However, due to individual immune responses, the virus remains dormant in the body and may only manifest when the immune system is compromised.
In other words, you need not worry about the risk of transmission, as it is possible that you are already a carrier without showing any symptoms.
Sincerely, Dr.

More Info
Recurrent herpes, particularly genital herpes, is primarily caused by the herpes simplex virus (HSV), which is most commonly transmitted through sexual activity. However, there are several other factors and potential causes for recurrent outbreaks that go beyond sexual contact. Understanding these can help in managing the condition and reducing the frequency of outbreaks.
1. Initial Infection: The first outbreak of herpes often occurs after the initial exposure to the virus, which can happen through non-sexual means as well. For instance, HSV can be transmitted through skin-to-skin contact with an infected area, even if the person does not have visible sores. This means that someone could contract the virus from a partner who may not be aware they are infected.
2. Immune System Factors: The immune system plays a crucial role in controlling the herpes virus. Factors that weaken the immune system, such as stress, illness, fatigue, or other infections, can trigger recurrent outbreaks. For example, conditions like the flu or even a common cold can lead to a resurgence of herpes symptoms.
3. Physical Triggers: Certain physical conditions can also provoke outbreaks. This includes factors such as:
- Hormonal Changes: Fluctuations in hormones, particularly during menstruation, can trigger outbreaks in some women.
- Sunlight Exposure: Excessive sun exposure can lead to outbreaks, particularly on the lips (cold sores), but can also affect genital herpes.
- Trauma or Injury: Physical trauma to the genital area, such as friction from sexual activity or injury, can also trigger an outbreak.
4. Lifestyle Factors: Lifestyle choices can impact the frequency of herpes outbreaks. Poor diet, lack of sleep, and high-stress levels can all contribute to a weakened immune response, making it easier for the virus to reactivate.
5. Other Medical Conditions: Certain medical conditions, such as diabetes or autoimmune disorders, can affect the immune system and increase the likelihood of herpes outbreaks.
6. Medications: Some medications that suppress the immune system, such as corticosteroids or chemotherapy drugs, can also lead to increased susceptibility to herpes outbreaks.
7. Genetic Factors: Some individuals may have a genetic predisposition that makes them more susceptible to recurrent herpes outbreaks.
In summary, while sexual activity is a primary mode of transmission for herpes simplex virus, there are numerous other factors that can lead to recurrent outbreaks. These include immune system health, physical triggers, lifestyle choices, and other medical conditions. If your boyfriend is experiencing recurrent herpes outbreaks, it may be beneficial for him to consult with a healthcare provider. They can offer guidance on managing the condition, including antiviral medications that can help reduce the frequency and severity of outbreaks, as well as lifestyle modifications that may help strengthen his immune system.
Additionally, it's important to maintain open communication about sexual health and practices, as well as to consider regular check-ups to monitor the condition and any potential complications.
