GLUCOSAMINE (Effectiveness and Side Effects in the Treatment of Knee Osteoarthritis Pain).
Can this prescription be issued by the hospital? Please respond.
Thank you.
KERRY, 20~29 year old female. Ask Date: 2001/04/19
Dr. Ye Qingnian reply Orthopedics
The efficacy of glucosamine is uncertain and it is likely to be removed from health insurance coverage, so our hospital does not offer it.
Currently, for patients with knee osteoarthritis pain, we only use analgesics and muscle relaxants.
For patients with moderate degeneration, if there is no improvement after six months of treatment, intra-articular injections of hyaluronic acid may be considered.
We also advise patients to avoid heavy lifting, reduce body weight, and for those with severe pain, using a cane to assist with walking.
As for a 25-year-old patient, it is unlikely to have osteoarthritis; if there is knee pain, further examination is needed to check for conditions such as chondromalacia patellae or other causes.

More Info
Glucosamine is a dietary supplement commonly used for the treatment of knee pain, particularly in cases of osteoarthritis. Its effectiveness and safety profile have been subjects of extensive research and debate.
Effectiveness of Glucosamine
Glucosamine is a naturally occurring compound found in cartilage, the tissue that cushions joints. It is believed to help in the repair and maintenance of cartilage, potentially alleviating pain and improving joint function in individuals suffering from osteoarthritis. Some studies suggest that glucosamine sulfate, in particular, may provide relief from knee pain and improve joint mobility. However, the results are mixed, with some clinical trials showing significant benefits while others indicate minimal to no improvement compared to placebo.
The effectiveness of glucosamine can vary based on individual factors such as the severity of joint degeneration, the specific formulation used (glucosamine sulfate vs. glucosamine hydrochloride), and the duration of treatment. Generally, it is recommended to take glucosamine for at least 6 to 8 weeks to assess its benefits fully.
Side Effects of Glucosamine
Glucosamine is generally considered safe for most people when taken at recommended doses. Common side effects may include gastrointestinal issues such as nausea, diarrhea, and heartburn. Some individuals may also experience allergic reactions, particularly those with shellfish allergies, as glucosamine is often derived from shellfish sources.
There have been concerns regarding the potential impact of glucosamine on blood sugar levels, particularly for individuals with diabetes. However, most studies have not found significant adverse effects on glucose metabolism. It is essential for individuals with pre-existing health conditions, especially those related to the heart or kidneys, to consult a healthcare provider before starting glucosamine.
Prescription Status
In many countries, including the United States, glucosamine is classified as a dietary supplement rather than a prescription medication. This means that it is available over-the-counter and does not require a doctor's prescription. However, healthcare professionals can recommend it as part of a comprehensive treatment plan for knee pain or osteoarthritis.
In clinical settings, physicians may prefer to prescribe traditional medications such as non-steroidal anti-inflammatory drugs (NSAIDs) or muscle relaxants for managing knee pain, particularly in acute cases. In cases where glucosamine is deemed appropriate, it may be suggested as an adjunct therapy alongside other treatments.
Recommendations
For individuals considering glucosamine for knee pain, it is advisable to:
1. Consult a Healthcare Provider: Before starting any new supplement, especially if you have underlying health conditions or are taking other medications.
2. Monitor Symptoms: Keep track of any changes in pain levels or side effects after starting glucosamine. It may take several weeks to notice any benefits.
3. Consider Lifestyle Modifications: Alongside glucosamine, weight management, physical therapy, and low-impact exercises can significantly improve joint health and reduce pain.
4. Be Cautious with Other Medications: If you are taking medications for conditions such as diabetes or hypertension, discuss with your doctor how glucosamine may interact with your treatment plan.
In summary, glucosamine may offer some benefits for individuals suffering from knee pain due to osteoarthritis, but its effectiveness can vary. It is generally safe with minimal side effects, and while it does not require a prescription, consulting a healthcare provider is recommended to ensure it fits within your overall treatment strategy.
