White spot lesions
Hello doctor, I have noticed small white bumps appearing on and around my penis, and they seem to have been there for quite some time.
I just became aware of them, and these white bumps have hair growing on them and extend onto the penis.
Is this normal? They are not painful or itchy.
Occasionally, there is one larger bump from which I have squeezed out a white substance.
Also, one of my testicles is larger than the other, but it's not very pronounced.
Is this normal? I've been wanting to ask this but didn't know where to turn.
Should I see a doctor?

Hello: 1.
Based on your description, it seems to be localized folliculitis or sebaceous gland inflammation causing the swelling.
2.
It is normal for one testicle to be larger than the other, as long as the difference is not significant.
However, if there is a considerable size discrepancy, it may be necessary to consider the possibility of a mass within the scrotum, such as a hydrocele, spermatocele, hernia, or even epididymitis.
A medical evaluation is required to determine the cause.
Sincerely, Dr.

When it comes to noticing unusual changes in the genital area, such as white bumps on the penis, it is essential to approach the situation with caution and awareness. The appearance of white bumps can be concerning, and understanding their nature is crucial for determining whether medical advice is needed.
Firstly, it is important to note that white bumps on the penis can arise from various causes. Some common benign conditions include:
1. Pearly Penile Papules: These are small, white, harmless bumps that typically appear around the corona of the glans (the head of the penis). They are a normal anatomical variant and do not require treatment.
2. Fordyce Spots: These are small, painless, pale bumps that are sebaceous glands without hair follicles. They are common and harmless.
3. Folliculitis: This is an inflammation of the hair follicles, which can lead to small, red or white bumps. It can occur due to irritation, infection, or blockage of the follicles.
4. Genital Warts: Caused by certain strains of the human papillomavirus (HPV), these can appear as small, flesh-colored or white bumps. They may require medical evaluation and treatment.
5. Pearly Penile Papules: These are small, white, harmless bumps that typically appear around the corona of the glans (the head of the penis). They are a normal anatomical variant and do not require treatment.
6. Sebaceous Cysts: These are small lumps under the skin that can occur anywhere on the body, including the genital area. They are usually harmless but can become infected.
Regarding the asymmetry of the testicles, it is normal for one testicle to be slightly larger or hang lower than the other. However, significant changes in size, shape, or consistency should be evaluated by a healthcare professional.
Given your description of the white bumps being present for some time, and the fact that you have noticed a larger bump that you have squeezed to release a white substance, it is advisable to seek medical attention. While some conditions are benign, others may require treatment or further investigation to rule out infections or sexually transmitted diseases.
When you visit a healthcare provider, they will likely conduct a physical examination and may ask about your medical history, any symptoms you are experiencing, and any sexual activity that may be relevant. This information will help them determine the appropriate course of action, which may include:
- Observation: If the bumps are determined to be benign, no treatment may be necessary.
- Topical Treatments: If there is an infection or irritation, topical medications may be prescribed.
- Further Testing: If there is suspicion of a sexually transmitted infection or other conditions, laboratory tests may be conducted.
In conclusion, while some white bumps on the penis can be normal and harmless, it is essential to consult a healthcare professional for an accurate diagnosis and peace of mind. Do not hesitate to seek medical advice, especially if you notice any changes in size, color, or if you experience discomfort. Your health and well-being are paramount, and addressing any concerns promptly is always the best approach.
