Is it a miscarriage?
Doctor, I usually have a luteal phase of only 10 to 11 days, and my menstrual cycle is consistently 29 days.
However, this month my luteal phase lasted 13 days (with temperatures ranging from 36.7 to 36.8°C), and on the 13th day, my temperature spiked to 37.1°C.
But the next day, my period started, which was 4 days late this month, and my temperature dropped suddenly to the low phase temperature of 36.5°C.
Does this situation indicate a possible miscarriage? Would a miscarriage come with any symptoms? If it were a miscarriage, should the menstrual blood contain clots? Thank you.
TJ, 20~29 year old female. Ask Date: 2004/11/27
Dr. Lü Lizheng reply Obstetrics and Gynecology
The response suggests that while it doesn't seem like a natural miscarriage, the possibility cannot be ruled out.
A blood test for pregnancy would provide a clearer confirmation.
It's important to note that a miscarriage may not always be accompanied by noticeable symptoms.
If a miscarriage were to occur, the menstrual flow would typically contain blood clots; however, there are many reasons for the presence of blood clots in menstrual flow, and they are not necessarily caused by a natural miscarriage.
Sincerely, Lu Li-Zheng.

Understanding early pregnancy signs can be quite complex, especially when considering the possibility of a miscarriage. Based on your description, it seems you experienced an extended luteal phase, indicated by a high basal body temperature (BBT) lasting 13 days, which is longer than your usual 10-11 days. This could suggest that your body was preparing for a potential pregnancy. However, the subsequent onset of your menstrual period, which was delayed by four days, indicates that you may not have been pregnant after all.
In a typical menstrual cycle, the luteal phase occurs after ovulation and before the start of menstruation. If fertilization does not occur, hormone levels (particularly progesterone) drop, leading to the shedding of the uterine lining, which is your menstrual period. The fact that your temperature dropped significantly after the onset of your period further supports this.
Regarding your concerns about miscarriage, it's important to note that natural miscarriages can occur, but they often present with specific symptoms. Common signs of a miscarriage include:
1. Bleeding: This can vary from light spotting to heavy bleeding. In some cases, the blood may contain clots or tissue.
2. Cramping: Many women experience cramping or pain in the lower abdomen, which can feel similar to menstrual cramps but may be more intense.
3. Loss of pregnancy symptoms: If you were experiencing early pregnancy symptoms (like nausea, breast tenderness, etc.), a sudden loss of these symptoms can be a sign of miscarriage.
However, not all miscarriages are accompanied by noticeable symptoms. Some women may have a missed miscarriage, where the embryo stops developing but the body does not immediately recognize it, leading to no immediate symptoms.
In your case, since you mentioned that your menstrual period arrived after a high temperature phase, it is more likely that you were not pregnant. If you had been pregnant and experienced a miscarriage, you would typically expect to see some form of bleeding that could include clots, especially if the miscarriage was more advanced.
If you are concerned about the possibility of a miscarriage or if you experience heavy bleeding, severe pain, or any other unusual symptoms, it is crucial to consult with a healthcare provider. They can perform tests, such as blood tests to measure hormone levels or an ultrasound, to provide clarity on your situation.
In summary, while your symptoms could suggest a potential early pregnancy, the arrival of your menstrual period indicates that you likely were not pregnant this cycle. If you have further concerns or if your menstrual patterns change significantly, it would be wise to seek medical advice for a thorough evaluation.
