My right ear feels strange?

Hello doctor, recently I've been feeling something unusual in my right ear.
When I close my mouth and pinch my nose to perform a Valsalva maneuver, I notice a rush of air towards my right ear, but I don't feel anything in my left ear.
Sometimes, when I breathe forcefully, it seems like there's also a rush of air in my right ear.
What could be the reason for this? Thank you for your response.

Xiao Zhang, 20~29 year old female. Ask Date: 2004/12/01

Dr. Ye Dawei reply Otolaryngology

The unusual sensations you're experiencing in your right ear, particularly the feeling of pressure or air movement when you perform actions like pinching your nose and blowing, can be attributed to several potential causes. These sensations often relate to the function of the Eustachian tube, which connects the middle ear to the back of the throat. This tube helps equalize pressure in the ear and drain fluid. When it is not functioning properly, it can lead to feelings of fullness, pressure, or even unusual sounds in the ear.

One common condition that could explain your symptoms is Eustachian tube dysfunction (ETD). ETD can occur due to various factors, including allergies, colds, sinus infections, or changes in altitude. Given your history of allergic rhinitis, it's possible that inflammation or congestion in your nasal passages could be affecting the Eustachian tube's ability to open and close properly, leading to the sensations you're describing.

Another possibility is that you might be experiencing a mild form of middle ear barotrauma, which can happen when there is a significant difference in pressure between the inside of the ear and the external environment. This can occur during activities such as flying, diving, or even during a cold or allergy flare-up when the Eustachian tube is blocked.

The fact that you feel air moving into your right ear but not your left suggests that the issue may be localized to the right side, possibly due to a blockage or dysfunction in the Eustachian tube on that side. This could also explain why you feel pressure or a sensation of fullness when you perform actions that typically help equalize ear pressure.

In terms of solutions, here are some steps you can take:
1. Nasal Decongestants: If your symptoms are related to allergies or nasal congestion, over-the-counter decongestants or antihistamines may help reduce swelling and improve Eustachian tube function.

2. Valsalva Maneuver: You mentioned trying to blow your nose while pinching it shut. This maneuver can help equalize pressure in the ears. However, be gentle to avoid damaging the eardrum.

3. Stay Hydrated: Drinking plenty of fluids can help thin mucus and promote drainage.

4. Warm Compress: Applying a warm compress to the affected ear may help alleviate discomfort and promote drainage.

5. Avoid Irritants: If you have allergies, try to avoid known triggers that could exacerbate your symptoms.

If your symptoms persist or worsen, or if you experience additional symptoms such as pain, significant hearing loss, or discharge from the ear, it is important to seek medical attention. An ear, nose, and throat (ENT) specialist can perform a thorough examination, possibly including audiometric testing or imaging studies, to determine the underlying cause of your symptoms and recommend appropriate treatment.

In summary, while your symptoms may be related to Eustachian tube dysfunction or other benign conditions, it is crucial to monitor your situation closely and consult a healthcare professional if you have any concerns or if your symptoms do not improve.
