Hepatitis B vaccine

I received the hepatitis B vaccine 7 years ago, and in a recent test, my HBsAg was negative and Anti-HBs was negative.
Does this indicate that the antibodies have disappeared? Is the hepatitis B vaccine considered lifelong immunity? Do I need to get revaccinated? What is Anti-HBc?

nancy, 30~39 year old female. Ask Date: 2000/11/17

Dr. Yang Guojun reply Internal Medicine

The protection from the hepatitis B vaccine lasts approximately ten years on average.
In your case, the effectiveness has diminished, so receiving another dose of the hepatitis B vaccine is recommended.

Hepatitis B vaccination is a critical public health measure aimed at preventing Hepatitis B virus (HBV) infection, which can lead to chronic liver disease, cirrhosis, and liver cancer. The vaccine is typically administered in a series of three doses, and it is known to be highly effective in generating an immune response that provides long-term protection against HBV. However, the question of whether booster doses are necessary after the initial vaccination series is a common concern.

In your case, you mentioned that you received the Hepatitis B vaccine seven years ago and that recent tests showed HBsAg (Hepatitis B surface antigen) as negative and Anti-HBs (Hepatitis B surface antibody) as negative. This indicates that you do not currently have an active Hepatitis B infection and that your body has not retained the antibodies that provide immunity against the virus. It is important to note that while the Hepatitis B vaccine is effective, the immunity it provides can diminish over time. Studies suggest that while many individuals retain protective antibodies for at least 10 to 15 years after vaccination, some may lose detectable levels of antibodies, leading to concerns about potential vulnerability to infection.

The concept of "lifelong immunity" is somewhat misleading. While the vaccine is designed to provide long-term protection, it does not guarantee that antibody levels will remain high indefinitely. In fact, it is not uncommon for individuals to show a decline in Anti-HBs levels over time, even if they remain protected against infection due to immune memory. This immune memory allows the body to respond rapidly if exposed to the virus, but it may not be reflected in antibody tests.

Regarding your question about whether you need to receive another dose of the vaccine, the general recommendation is that if you have lost detectable Anti-HBs and are at risk of exposure to Hepatitis B (for example, through certain occupations, travel, or lifestyle factors), it may be prudent to receive a booster dose. The Centers for Disease Control and Prevention (CDC) suggests that individuals who are at increased risk for Hepatitis B infection should consider getting revaccinated if their antibody levels are low or undetectable.

As for Anti-HBc, this refers to the Hepatitis B core antibody. It is an indicator of past or current infection with the Hepatitis B virus. The presence of Anti-HBc can indicate that a person has been exposed to the virus at some point, even if they are currently not infected (as indicated by negative HBsAg). In your case, since you mentioned that your Anti-HBc was negative, it suggests that you have not been exposed to the virus.

In summary, while the Hepatitis B vaccine provides strong protection, it is not guaranteed to last a lifetime for everyone. Given your negative Anti-HBs result, it would be advisable to consult with your healthcare provider about the possibility of receiving a booster dose, especially if you are at risk for Hepatitis B exposure. Regular monitoring of your Hepatitis B status and vaccination history is essential for maintaining your health and preventing potential infections.
