Will applying medication to acne scars help them heal?
Hello, Doctor.
I am 22 years old this year.
The acne scars and enlarged pores I developed during puberty make my face look noticeably different from others.
No matter how thick the foundation I apply, it cannot cover the pitted scars on my face.
Is laser resurfacing the only effective treatment for these scars? How is the cost calculated? I have previously undergone glycolic acid peels, but I seem to have a poor tolerance for Neostrata; my face always becomes red and itchy after applying it.
Are there any alternative medications I can use? Thank you for your answers! :-) I am currently worried about finding a job...

AHA (alpha hydroxy acid) peels only treat the very superficial layers of the skin, with the purpose of exfoliating the stratum corneum and not penetrating into the dermis.
For acne-induced indentations, it is necessary to perform dermabrasion (both traditional and laser) on the surrounding normal skin down to the dermis to achieve a superficial improvement of the indentations, rather than completely eliminating the scar.
It may take several sessions of dermabrasion to see satisfactory results, but it cannot be done indefinitely due to the skin's inherent thickness.
As for the cost of laser dermabrasion, it generally depends on the area being treated; for example, treating the cheeks and forehead may range from tens of thousands of NT dollars.
Additionally, about 30% of patients may experience a phenomenon known as "post-inflammatory hyperpigmentation," which typically appears three to four weeks after the procedure.
The darkening of the treated areas may take three to four months, or even up to a year, to gradually return to the original skin tone.
Therefore, it is essential to consider the potential complications before undergoing dermabrasion.
Lastly, I would like to remind you that there are currently no topical medications that can make scars disappear completely, including AHA.

Acne scars and pits can be a significant concern for many individuals, especially those who have experienced severe acne during their teenage years. At 22, you are not alone in feeling self-conscious about the appearance of your skin. The good news is that there are various treatment options available, including topical creams, chemical peels, and laser treatments, each with its own effectiveness and suitability depending on the type and severity of the scars.
Topical Creams
While topical creams can help improve the appearance of acne scars, they may not be as effective for deeper pits or scars. Ingredients such as retinoids, alpha hydroxy acids (AHAs), beta hydroxy acids (BHAs), and certain peptides can promote skin cell turnover, improve texture, and enhance overall skin tone. However, results can vary, and it often takes consistent use over several weeks or months to see noticeable improvements.
For example, retinoids (like tretinoin) can help in reducing the appearance of scars by promoting collagen production and skin renewal. However, they can also cause irritation, especially for those with sensitive skin. If you have had adverse reactions to certain products, it’s essential to consult with a dermatologist to find alternatives that suit your skin type.
Chemical Peels
Chemical peels, such as those using glycolic acid or salicylic acid, can help exfoliate the skin and improve its texture. They work by removing the top layer of dead skin cells, which can help reduce the appearance of superficial scars. However, deeper scars may require more intensive treatments. Since you mentioned having a negative reaction to certain products, it’s crucial to discuss your history with a dermatologist who can recommend a suitable peel that minimizes irritation.
Laser Treatments
Laser treatments, such as fractional laser therapy, are often considered the gold standard for treating deeper acne scars and pits. These treatments work by creating micro-injuries in the skin, which stimulates the body’s natural healing process and promotes collagen production. The results can be significant, leading to smoother skin texture and reduced visibility of scars.
The cost of laser treatments can vary widely depending on the type of laser used, the number of sessions required, and the geographic location of the clinic. On average, you might expect to pay anywhere from $500 to $2,500 per session. It’s advisable to consult with a qualified dermatologist or cosmetic surgeon to get a personalized treatment plan and cost estimate.
Alternative Treatments
If you have had adverse reactions to certain products like Niacinamide, it’s essential to explore other options. Some alternatives include:
1. Microneedling: This involves using fine needles to create micro-injuries in the skin, promoting collagen production and improving skin texture over time.
2. Dermal Fillers: For deeper pits, dermal fillers can provide immediate volume and smoothness, although they are temporary solutions that require maintenance.
3. Platelet-Rich Plasma (PRP): This treatment involves drawing your blood, processing it to concentrate the platelets, and injecting it back into the skin to promote healing and rejuvenation.
Conclusion
In summary, while creams can help improve the appearance of acne scars, they may not be sufficient for deeper pits. Laser treatments are often the most effective option, but they come with a higher cost. It’s essential to consult with a dermatologist to evaluate your skin and discuss the best treatment options tailored to your needs. They can also help you navigate any adverse reactions to products you’ve previously used, ensuring that you find a regimen that works for you without causing irritation. Remember, achieving clear skin is a journey, and with the right approach, you can significantly improve your skin's appearance.
