We inquired with the Eastern District Health Bureau, and they also do not have clarity on how to process the payment.
It would be best for you to ask the hospital that performed your surgery and applied for the catastrophic illness card to submit a request to the Health Bureau to see if they can handle it as a special case.
Additionally, the catastrophic illness card exempts you from co-payments, but you still need to pay the registration fee.
