Why does my penis often feel a slight aching pain?


Hello doctor, I would like to ask why I have been experiencing mild, intermittent pain in my penis lately.
The pain is very slight, and both my urine and penis appear normal.
Could this be related to nocturnal emissions (I sleep in tight jeans, which might be causing pressure) or masturbation? At the end of January, I visited Chimei Hospital to see a urologist regarding three small white spots on the external part of my foreskin (not on the part that retracts).
The spots are painless and itch-free, and the doctor said they are a normal phenomenon.
Since it was my first visit to a urologist, I was a bit flustered and couldn't locate the three white spots, which made me worry about affecting the diagnosis.
The doctor prescribed me a medication called "Gentamicin ointment." Could the application of this medication be related to the mild pain I feel in the area where I applied it? The pain is very slight and occurs intermittently.
