Serious Sexual Dysfunction: A Guide for Young Men - Urology

Severe sexual dysfunction?

I started masturbating in fifth grade, almost every two days without fail.
By the time I reached the second year of junior high, I noticed the duration was getting shorter, but the frequency didn't decrease.
In high school, I found that the firmness had decreased somewhat, and the frequency of masturbation reduced to about twice a week.
It wasn't until last summer that I noticed sometimes I would wake up without an erection; when I did have one, it would quickly become soft.
Sometimes, even after a few rubs, it would soften again, and my lower back would easily feel sore.
What should I do in this situation? I really don't know how to handle it.
Thank you!

Bangz, 10~19 year old female. Ask Date: 2005/02/15

Dr. Xu Weikai reply Urology

Hello, your situation may not necessarily indicate erectile dysfunction, and further examination is needed for a proper diagnosis.
At the age of 17, it is common to experience premature ejaculation due to heightened sensitivity.
Typically, erections occur very quickly but may also diminish rapidly.
Your symptoms could be a result of long-term self-stimulation, which may have become routine and rushed.
If you were to engage in actual sexual intercourse, your performance might improve.
Currently, there is no need to worry excessively, as it may only add unnecessary stress.
This is a response from Dr.
Hsu Wei-kai, Director of Urology at Taitung Hospital, Department of Health.

Sexual dysfunction in young men is a complex issue that can stem from various physical, psychological, and lifestyle factors. Your concerns about erectile function, libido, and the frequency of masturbation are not uncommon, and many young men experience similar issues at some point in their lives. Here’s a comprehensive overview of the factors that might contribute to your situation and some potential steps you can take.

Understanding Sexual Dysfunction
1. Erectile Dysfunction (ED): It’s important to note that erectile dysfunction can occur in younger men, and it is not solely an issue for older individuals. Factors contributing to ED can include psychological stress, anxiety, depression, and lifestyle choices such as diet, exercise, and substance use (including alcohol and drugs). In your case, the fact that you have noticed a decrease in the hardness of your erections and that they sometimes do not occur upon waking could indicate a psychological component, especially if you are experiencing anxiety about your sexual performance.

2. Masturbation Frequency: Regular masturbation is a normal part of sexual health. However, excessive masturbation can sometimes lead to temporary changes in sexual function, including reduced sensitivity or difficulty achieving an erection. It’s essential to find a balance that feels right for you. If you feel that your masturbation habits are interfering with your sexual health or relationships, it might be worth considering reducing the frequency.

3. Physical Health Factors: Conditions such as obesity, diabetes, hormonal imbalances (like low testosterone), and cardiovascular issues can affect sexual function. If you have concerns about your physical health, it may be beneficial to consult with a healthcare provider for a thorough evaluation.

4. Psychological Factors: Anxiety, particularly performance anxiety, can significantly impact sexual function. If you find yourself worrying about your ability to perform sexually, this can create a cycle of anxiety that further exacerbates the problem. Techniques such as mindfulness, relaxation exercises, or therapy can be helpful in managing anxiety.

5. Lifestyle Choices: Factors such as diet, exercise, sleep, and substance use play a crucial role in sexual health. Regular physical activity, a balanced diet, and adequate sleep can improve overall health and, by extension, sexual function. Reducing alcohol consumption and avoiding recreational drugs can also be beneficial.

Steps to Take
1. Consult a Healthcare Provider: It’s essential to speak with a healthcare professional who can provide a comprehensive assessment of your sexual health. They can rule out any underlying medical conditions and discuss potential treatment options.

2. Consider Therapy: If anxiety or stress is a significant factor, speaking with a therapist or counselor can help you develop coping strategies and address any psychological barriers to sexual function.

3. Practice Healthy Habits: Focus on maintaining a healthy lifestyle. Regular exercise, a balanced diet, and good sleep hygiene can improve your overall well-being and sexual health.

4. Educate Yourself: Understanding sexual health can alleviate some anxiety. Reading reputable sources or speaking with a healthcare provider can help you gain a better understanding of what is normal and what might be a cause for concern.

5. Be Patient: Changes in sexual function can take time to address. Be patient with yourself as you explore these issues and work towards improvement.

Conclusion
Sexual dysfunction is a multifaceted issue that can be influenced by a variety of factors. It’s crucial to approach this topic with an open mind and seek professional guidance. By addressing both the physical and psychological aspects of your sexual health, you can work towards regaining confidence and improving your sexual function. Remember, you are not alone in this experience, and help is available.
