Issues with the glans penis, urethral meatus, and coronal sulcus?
Hello Doctor, I noticed some small red spots near the urethral opening of my glans.
I'm not sure what they are; they don't hurt, itch, or discharge anything.
What could this be? Could it be a bacterial infection? Also, I have two rows of small white papules on my corona.
I'm not sure what they are either; they also don't hurt, itch, or discharge anything.
I've done a lot of research online and heard that this can be normal, but I'm still quite skeptical.
I have sexual experience with only one regular partner.
Could you please help me with these concerns? Thank you!

Dr. Cai Bingkun reply Urology
Hello! For dermatological issues, it is best to visit a clinic for an in-person consultation, as seeing is believing.
The small white nodules in the coronal sulcus are normal structures known as pearly penile papules.
Thank you for your question!

When it comes to red spots and white bumps on the penis, it’s essential to approach the situation with a clear understanding of potential causes and implications. Based on your description, the red spots near the urethral opening that are painless and not discharging any fluid could be benign. They might represent a variety of conditions, including:
1. Pearly Penile Papules: These are small, harmless bumps that can appear around the corona of the glans (the head of the penis). They are a normal anatomical variant and are not associated with any disease or infection. Many men have them, and they typically do not require treatment.
2. Folliculitis: This is an inflammation of the hair follicles, which can sometimes occur in the genital area. It may present as small red bumps but usually comes with some discomfort or itchiness, which you have not reported.
3. Fordyce Spots: These are small, painless, pale bumps that are sebaceous glands without hair follicles. They are completely normal and common in many men.
4. Irritation or Allergic Reaction: Sometimes, irritation from soaps, lotions, or even friction can cause temporary redness or bumps. Given that you mentioned no pain or itching, this is less likely.
5. Infection: While bacterial infections can cause redness and discharge, the absence of pain, itching, or discharge makes a significant infection less likely. However, sexually transmitted infections (STIs) can sometimes present with atypical symptoms, so it’s essential to consider your sexual history.
Regarding the white bumps you mentioned on the corona, if they are similar to the descriptions above (like pearly penile papules or Fordyce spots), they are typically harmless. However, if there are changes in size, color, or if they become painful or start to discharge, it would be prudent to seek medical advice.
Recommendations:
1. Observation: If the spots and bumps are not causing any discomfort, it may be best to monitor them for any changes over time.
2. Consultation: If you have concerns, especially given your sexual history, it’s advisable to consult a urologist or a healthcare provider specializing in sexual health. They can perform a physical examination and may recommend tests if necessary.
3. Hygiene Practices: Maintain good hygiene, but avoid excessive washing or scrubbing, as this can lead to irritation. Use mild, unscented soaps to minimize the risk of allergic reactions.
4. Safe Practices: Continue practicing safe sex, and consider regular STI screenings, especially if you have multiple partners or if your partner has had previous partners.
5. Education: Understanding that many variations in anatomy are normal can help alleviate anxiety. However, being informed about your body and any changes is crucial for your health.
In conclusion, while the symptoms you describe are likely benign, it’s always best to err on the side of caution and consult a healthcare professional if you have any doubts or if the situation changes. Regular check-ups and open communication with your healthcare provider can help ensure your sexual health remains a priority.
