Mild dull pain in the lower right abdomen?

Dear Doctor, I am currently nine weeks pregnant.
Yesterday, I almost fell down the stairs, but I managed to grab onto something in time.
However, my body was shaken, especially my lower half.
Today, I have been feeling a mild discomfort in my lower right abdomen all day, and I noticed that my usual morning sickness has not occurred today.
Will this affect the fetus? This is my first pregnancy, and I have read many cases of stillbirth, which makes me very worried.
If a stillbirth were to occur, would the mother have any sensations? What are the signs to look out for?

worry, 20~29 year old female. Ask Date: 2005/02/27

Dr. Lü Lizheng reply Obstetrics and Gynecology

The mild dull pain in the lower right abdomen may be due to uterine contractions.
Resting more will likely help it resolve on its own, and it usually does not affect the fetus.
If there is fetal demise, the mother may not feel any symptoms, and an ultrasound is necessary to confirm.
Sincerely, Lu Li-Cheng.

During early pregnancy, it is common for women to experience various symptoms, including mild abdominal pain. In your case, you mentioned experiencing mild discomfort in the right lower abdomen after a near fall from the stairs. It's understandable to be concerned about the potential impact on your pregnancy, especially as a first-time mother.

Firstly, it's important to recognize that mild abdominal pain can occur for various reasons during pregnancy. Some of these reasons include the stretching of ligaments as the uterus expands, which is a normal part of pregnancy. This type of pain is often described as a dull ache or a feeling of pressure and can be more pronounced during activities that involve sudden movements or physical exertion.

However, since you experienced a physical jolt from the near fall, it is wise to monitor your symptoms closely. If the pain persists, worsens, or is accompanied by other concerning symptoms such as heavy bleeding, severe cramping, or fever, it is crucial to seek medical attention immediately. Your healthcare provider may perform an ultrasound or other examinations to ensure that everything is progressing normally with your pregnancy.

Regarding your concern about the absence of typical pregnancy symptoms like nausea, it is not uncommon for symptoms to fluctuate during pregnancy. Some women may experience a decrease in nausea or other symptoms at different stages, and this does not necessarily indicate a problem. However, if you notice significant changes in your symptoms or have concerns about the health of your baby, it is always best to consult your healthcare provider for reassurance.

As for the fear of fetal demise, it is important to understand that while it can happen, it is relatively rare, especially in the early stages of pregnancy. If a fetus were to pass away, some women may experience symptoms such as a decrease in pregnancy symptoms (like nausea or breast tenderness), changes in fetal movement (though this is more applicable in later stages), or unusual abdominal pain. However, many women may not experience any noticeable symptoms, which can add to the anxiety.

In summary, while mild abdominal pain can be a normal part of early pregnancy, especially after a physical incident, it is essential to remain vigilant and communicate with your healthcare provider about any concerns. Regular prenatal check-ups and open communication with your doctor can help alleviate worries and ensure the health of both you and your baby. If you have any doubts or if your symptoms change, do not hesitate to reach out to your healthcare provider for guidance and support. Remember, your health and peace of mind are paramount during this exciting yet sometimes anxious time.
