Is the probability of getting pregnant high?

Hello! My first child is already a year and a half old, and my husband and I really want to have a second child, but we haven't had any luck so far.
I would like to ask about my menstrual cycle in February, which started on the 22nd (my cycles are usually 28-30 days).
We tried to conceive on March 4, 5, 7, and 9.
What are the chances of getting pregnant with this timing? Also, after each time we try, I lie flat and notice some fluid coming out; is that semen? Will this affect the chances of conception? Thank you!

Jinzhang de nüren, 30~39 year old female. Ask Date: 2005/03/11

Dr. Li Guanghan reply Obstetrics and Gynecology

Hello: Please use basal body temperature or ovulation test strips to identify your ovulation period, and then engage in intercourse, as this increases the chances of success.
After intercourse, please lie down with your hips elevated for 20 minutes before getting up; it is normal for semen to leak out.

More Info

To enhance your chances of conceiving, it's essential to understand the timing of ovulation, the fertility window, and some practical tips that can help optimize your chances of pregnancy. Based on your menstrual cycle, which typically ranges from 28 to 30 days, ovulation usually occurs around 14 days before your next period. Given that your last menstrual period (LMP) started on February 22, you would likely ovulate around March 7 to March 9. Engaging in intercourse on March 4, 5, 7, and 9 aligns well with your fertile window, which is a positive sign for increasing your chances of conception.

Understanding the Fertile Window
The fertile window is the period during which intercourse is most likely to result in pregnancy. This window typically spans about six days: the five days leading up to ovulation and the day of ovulation itself. Sperm can survive in the female reproductive tract for up to five days, which means that having intercourse in the days leading up to ovulation can increase the likelihood of sperm being present when the egg is released.

Factors Affecting Conception
1. Timing: As mentioned, timing intercourse around ovulation is crucial. Since you have already timed your efforts well, continue to monitor your cycle for any changes.

2. Health and Lifestyle: Maintaining a healthy lifestyle can significantly impact fertility. This includes a balanced diet, regular exercise, avoiding smoking and excessive alcohol consumption, and managing stress levels.

3. Medical Conditions: If you have any underlying health issues, such as polycystic ovary syndrome (PCOS) or thyroid disorders, these can affect your fertility. Consulting with a healthcare provider can help address any concerns.

4. Age: Fertility naturally declines with age, particularly after the age of 35. If you are in this age range, it may take longer to conceive.

Addressing Concerns About Semen Leakage
Regarding your concern about fluid leaking out after intercourse, it is common for some semen to flow out of the vagina after ejaculation. This does not necessarily affect your chances of conception. The majority of sperm are deposited near the cervix, and even if some semen leaks out, there are still plenty of sperm that can swim through the cervical mucus to reach the egg.
To maximize the chances of conception after intercourse, some women choose to lie down for about 15-20 minutes to allow gravity to assist in keeping the sperm near the cervix, although this is not strictly necessary.

Additional Tips for Boosting Fertility
- Track Ovulation: Consider using ovulation predictor kits or tracking basal body temperature to pinpoint ovulation more accurately.

- Healthy Weight: Both underweight and overweight can affect hormone levels and ovulation. Aim for a healthy weight through diet and exercise.

- Supplements: Some women find it beneficial to take prenatal vitamins or supplements like folic acid, which is important for fetal development.

- Consult a Specialist: If you have been trying to conceive for over a year without success (or six months if you are over 35), it may be time to consult a fertility specialist for further evaluation.

In summary, your timing appears to be appropriate for increasing your chances of pregnancy. Focus on maintaining a healthy lifestyle, tracking your ovulation, and addressing any medical concerns with your healthcare provider. Good luck on your journey to expanding your family!
