Hepatitis B health management medications

Dear Dr.
Lai,
I am a patient with hepatitis B and have been taking various so-called liver protection medications for several decades.
In recent years, I switched to using "Sisu Liver Protection Gel" from Wuzhou Pharmaceutical.
I recently visited a local medical laboratory for a routine check-up, and everything was normal except for the hepatitis B issue.
During my conversation with the lab technician, I was informed that taking the aforementioned Sisu Liver Protection Gel was incorrect.
They also recommended a product containing "Yexiazhu" as an alternative (which is sold as a food product, not a medication).
Could you please provide me with more accurate information regarding liver health medications for hepatitis B? Thank you very much.

Gao R, 50~59 year old female. Ask Date: 2005/04/17

Dr. Lai Shuyi reply Pharmacology

Hello: Silymarin Liver Protection Capsules contain silymarin and some vitamins.
Silymarin is a plant-derived compound that stabilizes liver cell membranes, promotes bile secretion to accelerate toxin elimination, and reduces abnormal lipid changes in the liver and gallbladder, facilitating lipid metabolism.
The indications approved by the Department of Health include chronic liver disease, cirrhosis, and fatty liver as an adjunctive treatment.

Hepatitis B (HBV) is a significant public health concern worldwide, and understanding the treatment options and supplements available for patients is crucial for effective management. As a patient with chronic Hepatitis B, it is essential to be well-informed about the medications and supplements you are taking, as well as their potential effects on your liver health.

Firstly, it is important to clarify that while there are various supplements marketed for liver health, not all of them are scientifically validated or approved as effective treatments for Hepatitis B. The use of herbal or dietary supplements, such as the "斯斯保肝膠蘘" (which translates to "Sisi Liver Protection Gel"), can be misleading. Many patients believe that these products can help manage their condition, but they often lack rigorous clinical evidence to support their efficacy. In fact, some supplements may even interfere with conventional antiviral treatments or exacerbate liver damage.

The primary treatment options for chronic Hepatitis B include antiviral medications such as entecavir and tenofovir. These medications are well-studied and have been shown to effectively suppress the virus, reduce liver inflammation, and prevent the progression to cirrhosis or liver cancer. It is crucial to adhere to the prescribed antiviral therapy and to have regular follow-ups with your healthcare provider to monitor liver function and viral load.

Regarding the suggestion of using a product containing "葉下珠" (which translates to "Corydalis"), it is essential to approach such recommendations with caution. While some herbal ingredients may have traditional uses in liver health, their safety and efficacy in the context of Hepatitis B are not well-established. Patients should always consult their healthcare provider before starting any new supplement, especially if they are already on antiviral therapy.

In terms of lifestyle modifications, patients with Hepatitis B should focus on maintaining a healthy diet, avoiding alcohol, and managing any coexisting conditions such as diabetes or obesity, which can further strain liver function. Regular exercise and weight management can also contribute positively to overall liver health.

It is also vital to understand the importance of vaccination against Hepatitis A, as co-infection with Hepatitis A can lead to more severe liver disease in patients with Hepatitis B. Additionally, practicing safe behaviors to prevent transmission of Hepatitis B to others is crucial, including avoiding sharing personal items that may come into contact with blood.

In summary, while there are many products marketed for liver health, it is essential to rely on evidence-based treatments for managing Hepatitis B. Antiviral medications remain the cornerstone of treatment, and patients should be cautious about using supplements without consulting their healthcare provider. Regular monitoring and lifestyle adjustments can significantly impact the long-term health outcomes for individuals living with Hepatitis B. Always prioritize communication with your healthcare team to ensure that you are making informed decisions about your health.
