Am I experiencing erectile dysfunction?
Hello, Dr.
Hsu.
My issue began about a year ago when I was living a very unhealthy lifestyle, smoking heavily (two packs a day), staying up late, and getting very little sleep.
At that time, my sexual function was normal, but I felt a lot of pressure and even some pain during erections.
I frequently watched adult films and often engaged in masturbation, almost daily.
After losing my job, I even did it 2-3 times a day.
There were a few instances where I couldn't urinate after masturbation, and a few times I was unable to achieve an erection or my erection quality significantly decreased.
Due to my introverted personality and living alone, I spent my days at home reading newspapers and browsing the internet, rarely going out.
I felt very anxious and stressed, and I coped by overeating.
I gained nearly 25 kg in six months.
I now know I have high blood pressure and take antihypertensive medication daily.
My sexual function has deteriorated; I can achieve an erection, but the hardness is only about 50-60% of what it used to be.
I want to change my bad habits and face life anew, hoping to regain my sexual function and health.
I understand I need to visit a hospital for treatment, but I would like to hear your preliminary thoughts based on my description to have a better understanding of my situation.
Thank you sincerely.

Hello: In response to your question, given your age, it is unlikely that you have organic erectile dysfunction.
It is more likely related to your smoking, hypertension, and psychological issues.
You may want to seek help from a relevant physician at an outpatient clinic.

More Info
Erectile dysfunction (ED) is a complex condition that can be influenced by a variety of physical, psychological, and lifestyle factors. Based on your description, it appears that you have experienced a significant change in your sexual health over the past year, which may be attributed to several interrelated causes.
Causes of Erectile Dysfunction
1. Lifestyle Factors: Your previous lifestyle, characterized by heavy smoking (two packs a day), poor sleep habits, and high levels of stress and anxiety, can significantly impact erectile function. Smoking is known to impair blood flow and can lead to vascular problems, which are critical for achieving and maintaining an erection. Additionally, the increase in body weight (25 kg in six months) can contribute to hormonal imbalances and further exacerbate ED.
2. Psychological Factors: The feelings of anxiety, depression, and isolation you described can also play a significant role in erectile dysfunction. Psychological factors can lead to performance anxiety, which can create a cycle of dysfunction. The fact that you were engaging in frequent masturbation (DIY) may have also contributed to a psychological reliance on this behavior, potentially leading to difficulties in achieving erections in other contexts.
3. Medical Conditions: You mentioned that you have high blood pressure and are on medication for it. Hypertension can affect erectile function, as it can damage blood vessels and reduce blood flow to the penis. Additionally, certain medications for high blood pressure can also have side effects that contribute to ED.
Symptoms of Erectile Dysfunction
The symptoms you are experiencing, such as reduced hardness of erections (5-6 out of 10), difficulty achieving an erection, and instances of urinary retention after masturbation, are indicative of erectile dysfunction. These symptoms can be distressing and can impact your quality of life and self-esteem.
Solutions and Recommendations
1. Lifestyle Changes: The first step towards improving your erectile function is to adopt a healthier lifestyle. This includes quitting smoking, reducing alcohol intake, maintaining a balanced diet, and engaging in regular physical activity. Weight loss can also improve erectile function, especially if you are currently overweight.
2. Stress Management: Since anxiety and stress are significant contributors to your condition, finding effective ways to manage stress is crucial. This may include mindfulness practices, meditation, or engaging in hobbies that you enjoy. Consider seeking support from a mental health professional if feelings of anxiety or depression persist.
3. Medical Evaluation: It is essential to consult with a healthcare provider for a comprehensive evaluation. They can assess your overall health, review your medications, and possibly adjust them if they are contributing to your ED. Additionally, they may conduct tests to rule out other underlying conditions that could be affecting your sexual health.
4. Therapeutic Options: Depending on the underlying causes identified, your doctor may recommend various treatment options for ED. These can include oral medications (such as phosphodiesterase type 5 inhibitors), vacuum erection devices, or even counseling for psychological factors.
5. Regular Follow-ups: Since you are already aware of the need to seek medical attention, make sure to follow up regularly with your healthcare provider to monitor your progress and make necessary adjustments to your treatment plan.
Conclusion
Erectile dysfunction can be a challenging condition, but it is important to remember that it is treatable. By addressing lifestyle factors, managing stress, and seeking medical advice, you can work towards improving your sexual health and overall well-being. It is commendable that you are motivated to make positive changes in your life, and taking these steps can lead to significant improvements in your erectile function and quality of life.
