Itchy foreskin?
Hello Doctor: Recently, I noticed many small white bumps (very tiny) inside my foreskin, concentrated together and very itchy.
I have not had sexual intercourse, but I occasionally masturbate.
What could this condition be? Can it be treated? Thank you!

Dear Mr.
Yuxu,
Hello, you mentioned having small white spots on your foreskin? This is a common situation and can be difficult to diagnose precisely.
I recommend that you have it examined in person.
Alternatively, you could take a digital photo to provide for reference.
My guess is that it may be a localized infection.
However, I still advise you to enhance your local hygiene practices.
Avoid using soaps or other cleansing agents, as the skin in this area is not true skin but rather mucous membrane (similar to the lining of the mouth), which is delicate and easily injured.
Therefore, you should simply retract the foreskin and rinse with clean water.
If it is particularly dirty, you may use a body wash, especially one that emphasizes moisturizing properties, like a cream-based product that leaves a slippery feeling after washing, indicating it may not feel completely clean.
Absolutely avoid using cleansing products that leave the area feeling overly dry after washing.
Sincerely,
Dr.

More Info
Itchy white bumps under the foreskin can be concerning, and it's important to understand the potential causes and treatments available. Based on your description, these bumps could be due to several conditions, including but not limited to:
1. Pearly Penile Papules: These are small, harmless bumps that can appear around the corona of the glans (the head of the penis). They are normal anatomical variations and do not require treatment unless they cause discomfort or distress.
2. Fordyce Spots: These are small, painless, pale bumps that are actually enlarged sebaceous glands. They are common and harmless, often appearing on the shaft of the penis or the scrotum.
3. Balanitis: This is an inflammation of the glans and can be caused by poor hygiene, infections (bacterial or fungal), or irritants. Symptoms often include redness, swelling, and itching, along with the appearance of white bumps or patches.
4. Genital Warts: Caused by the human papillomavirus (HPV), these can appear as small, flesh-colored or white bumps. They may be itchy and can spread through sexual contact.
5. Lichen Sclerosus: This is a chronic skin condition that can affect the genital area, leading to white patches, itching, and discomfort. It requires medical evaluation and treatment.
6. Candidiasis (Yeast Infection): This fungal infection can cause itching and white patches or bumps under the foreskin, especially if there is an imbalance in the normal flora.
Given that you have not engaged in sexual intercourse but have experienced itching and the appearance of these bumps, it is crucial to consider hygiene practices and any potential irritants. Regular cleaning of the genital area is essential, as is avoiding harsh soaps or irritants that could exacerbate the condition.
Treatment Options
1. Good Hygiene: Keeping the area clean and dry is fundamental. Gently wash the area with mild soap and water, and ensure it is thoroughly dried afterward.
2. Topical Treatments: Depending on the diagnosis, over-the-counter antifungal creams may help if a yeast infection is suspected. If it is an inflammatory condition, a topical corticosteroid may be recommended by a healthcare provider.
3. Avoid Irritants: If you suspect that certain soaps, lotions, or other products are causing irritation, discontinue their use.
4. Medical Evaluation: It is essential to consult a healthcare provider for an accurate diagnosis. They may perform a physical examination and, if necessary, take a sample for further testing to rule out infections or other conditions.
5. Follow-Up: If the bumps persist or worsen, or if you experience additional symptoms such as pain, discharge, or bleeding, seek medical attention promptly.
In conclusion, while some causes of itchy white bumps under the foreskin are benign, others may require treatment. It is advisable to consult with a healthcare professional who can provide a thorough examination and appropriate management based on your specific condition. Remember, early diagnosis and treatment can prevent complications and provide relief from symptoms.
