Menstruation lasts continuously for 1 to 7 days?
Hello Doctor: In March, I was diagnosed with pregnancy by an obstetrician and underwent a medical abortion using RU486.
However, due to incomplete expulsion, I had a dilation and curettage (D&C) under local anesthesia to ensure everything was cleared.
Now, I have a question: my menstrual periods in April and May came about 1-6 days late and seemed to be almost finished, but then I experienced irregular light bleeding afterward.
I would like to know if this is normal? The light bleeding is bright red in color.
P.S.
I have been attending all my follow-up appointments as scheduled.
This situation is making me quite anxious.
Thank you, Doctor.

Dr. Zhong Wenzhen reply Obstetrics and Gynecology
Menstrual bleeding lasting from 1 to 7 days is generally considered to be within the normal range; however, any subsequent light bleeding is not normal.
Possible causes include persistent bleeding from small blood vessels in the endometrium or incomplete healing of the endometrium (both of which are essentially the same event).
At this point, it is advisable to consult a physician for diagnosis and treatment.
Wishing you good health.

After an abortion, it is not uncommon to experience irregular menstrual bleeding, which can include variations in flow, duration, and color. Your situation, where you had a medical abortion using RU-486 followed by a dilation and curettage (D&C) procedure due to incomplete abortion, is a context where such irregularities can occur.
Firstly, it is important to understand that after an abortion, the body undergoes significant hormonal changes as it returns to its pre-pregnancy state. This process can take time, and during this period, menstrual cycles may not be regular. The fact that your periods in April and May were shorter than usual (1-6 days) is not necessarily alarming, especially if you have been following up with your healthcare provider and they have not indicated any issues.
The occurrence of spotting or light bleeding after your menstrual period can be attributed to several factors:
1. Hormonal Fluctuations: After an abortion, your hormone levels, particularly estrogen and progesterone, may take some time to stabilize. This can lead to irregular bleeding patterns, including spotting between periods.
2. Uterine Healing: Following a D&C, the uterus needs time to heal. During this healing process, it is possible to experience light bleeding or spotting as the uterine lining regenerates.
3. Residual Tissue: In some cases, if there is any remaining tissue in the uterus after the abortion, it can lead to irregular bleeding. However, since you mentioned that you had a follow-up and the procedure was done to ensure everything was cleared, this is less likely to be the cause.
4. Infection: Although less common, an infection following an abortion can cause abnormal bleeding. If you experience symptoms such as fever, severe abdominal pain, or foul-smelling discharge, it is essential to seek medical attention promptly.
5. Other Factors: Stress, changes in weight, and lifestyle factors can also affect your menstrual cycle. If you have been under stress or have experienced significant changes in your routine, this could contribute to irregular bleeding.
Regarding the color of the blood, fresh red blood typically indicates active bleeding, which can happen during the menstrual cycle or due to other factors like hormonal changes. If the bleeding is light and not accompanied by severe pain or other concerning symptoms, it may not be a cause for alarm.
However, it is crucial to monitor your symptoms closely. If the irregular bleeding persists, becomes heavier, or is accompanied by other symptoms such as pain or fever, you should contact your healthcare provider for further evaluation. They may recommend additional tests, such as an ultrasound, to ensure that everything is healing properly and that there are no complications.
In summary, while irregular bleeding after an abortion can be concerning, it is often a normal part of the recovery process. Maintaining open communication with your healthcare provider and attending follow-up appointments will help ensure that any potential issues are addressed promptly. If you continue to feel anxious about your symptoms, do not hesitate to discuss your concerns with your doctor, as they can provide reassurance and guidance tailored to your specific situation.
