Unusual Urine Odors: Is It Normal or a Medical Concern? - Urology

Is there a problem?

Dear Dr.
Hsu,
I have noticed that when I urinate, there is a smell reminiscent of semen.
Perhaps some urine stains have been left behind, causing my pants to also have an odor, which is quite embarrassing.
Is this what is referred to as "nocturnal emission"? Is it common for people to experience this? Additionally, I wonder if straining my abdomen or holding in my urine contributes to this situation.
I have noticed that sometimes when I stretch my abdomen, there is a sensation as if "something is coming out" from my urethra.
The urine expelled when I hold it in sometimes appears slightly cloudy and carries a semen-like odor.
Is this what is known as "spermatic retention"? Should I seek treatment for this?

Xiao Yuan, 20~29 year old female. Ask Date: 2005/05/16

Dr. Xu Weikai reply Urology

Xiao Yuan: Hello, based on your description, it may just be a common infection.
As for the issue of "solidification of semen," this is not something that Western medicine places much emphasis on.
Instead, it is considered a normal phenomenon that will naturally resolve itself when the time is right.
There is no need for excessive concern.
Recommendations: 1.
Conduct a urinalysis to check for any issues and treat them accordingly.
2.
Avoid making unfounded assumptions.
Director of Urology, Xu Wei-Kai, responds on behalf of the Taitung Hospital of the Department of Health, Executive Yuan, and expresses concern for you.

When it comes to unusual urine odors, such as the one you described that resembles semen, it can indeed be concerning and warrants further investigation. While it is not uncommon for urine to have different odors based on diet, hydration levels, and other factors, a persistent or unusual smell can indicate an underlying medical issue.

Firstly, the odor you are experiencing could be related to several factors. One possibility is that it could be due to the presence of certain substances in your urine, which can be influenced by your diet. Foods such as asparagus, garlic, or certain spices can alter the smell of urine. Additionally, dehydration can concentrate urine and intensify its odor. However, if the smell is reminiscent of semen and is accompanied by other symptoms, it may be indicative of a more serious condition.

The sensation you described of feeling something "flowing out" when you exert yourself or hold your urine could also be a sign of a urinary tract issue. This sensation might be linked to involuntary leakage or a condition known as stress urinary incontinence, where physical activities such as coughing, sneezing, or exertion lead to involuntary urine loss. However, if you are experiencing a white, cloudy discharge along with the unusual odor, this could suggest an infection or another medical condition that requires attention.

In terms of "关精不固" (which can be translated to "failure to retain essence"), this concept is rooted in traditional Chinese medicine and refers to a condition where there is a lack of control over bodily fluids, often associated with kidney or reproductive health issues. While this terminology may not directly correlate with Western medical diagnoses, it does highlight the importance of addressing any symptoms related to urinary function and fluid retention.

Given your symptoms, it is advisable to consult a healthcare professional for a thorough evaluation. A urinalysis can help determine if there are any infections, the presence of white blood cells, or other abnormalities in your urine. If an infection is present, appropriate antibiotics can be prescribed. Additionally, if there are concerns regarding your prostate health or other reproductive organs, further examinations such as an ultrasound or prostate-specific antigen (PSA) test may be warranted.

In summary, while occasional changes in urine odor can be normal, persistent or unusual odors, especially when accompanied by other symptoms, should not be ignored. It is essential to seek medical advice to rule out any potential infections or underlying health issues. Early intervention can prevent complications and ensure that any necessary treatment is administered promptly.
