What are the side effects of birth control pills?

Is it true that taking birth control pills can cause weight gain? I've also heard about a type of birth control pill called "Qlaira" or "Diane" that can be used to treat acne.
Is that correct? Is there a way to take birth control pills without gaining weight?

Dr. Zhong Wenzhen reply Obstetrics and Gynecology

Hello, Xiaoru: The truth about taking birth control pills is that weight may increase, but it does not mean gaining fat.
This is due to the action of progesterone, which retains 0.5 to 3 kilograms of water in the body, including in the breasts (causing swelling), the uterus (leading to a feeling of heaviness in the lower abdomen), and the skin (resulting in a glowing appearance and reduced wrinkles).
After taking hormones for 21 days, the retained water is typically expelled from the body within about three days, so at this point, weight should return to the state it was before using the birth control pills.
If there is still a slight increase in weight at this time, it may be due to an increase in fat rather than a side effect of the birth control pills.
Newer generations of oral contraceptives claim that the newly synthesized progesterone has better receptor activity without the androgenic effects, which theoretically reduces the stimulation of acne formation.
It may even have a therapeutic effect on acne.

The question of whether birth control pills cause weight gain is a common concern among women considering or currently using hormonal contraceptives. The relationship between birth control pills and weight gain is often surrounded by myths and misconceptions, so it’s essential to clarify the facts.

Myths and Facts about Birth Control Pills and Weight Gain
1. Myth: All Birth Control Pills Cause Weight Gain
Fact: Research has shown that the majority of women do not experience significant weight gain as a result of taking birth control pills. While some studies have reported slight weight changes, these are often minimal and not clinically significant. The perception that birth control pills lead to weight gain may stem from anecdotal experiences rather than scientific evidence.

2. Myth: Weight Gain is Inevitable with Birth Control Pills
Fact: Weight gain is not a guaranteed side effect of birth control pills. Individual responses to hormonal contraceptives can vary widely. Factors such as genetics, lifestyle, diet, and overall health play a more substantial role in weight management than the contraceptive method itself.
3. Myth: Certain Pills Cause More Weight Gain than Others
Fact: Some formulations of birth control pills may have different side effects, including potential changes in weight. For example, pills containing higher doses of estrogen may lead to fluid retention, which can cause temporary weight gain. However, newer formulations often contain lower doses of hormones and are less likely to cause this effect.

4. Myth: Birth Control Pills Can Help with Acne
Fact: Some birth control pills, particularly those containing estrogen and progestin, can indeed help manage acne. Medications like "Diane-35" (also known as "Diane" or "Yasmin") are often prescribed for this purpose. They work by regulating hormones that can contribute to acne development. However, it’s essential to consult with a healthcare provider to determine the best option based on individual health needs.

5. Managing Weight While on Birth Control
If you are concerned about weight gain while taking birth control pills, there are several strategies you can adopt:
- Healthy Diet: Focus on a balanced diet rich in fruits, vegetables, whole grains, and lean proteins. Avoid excessive consumption of processed foods and sugars.

- Regular Exercise: Incorporate physical activity into your routine. Aim for at least 150 minutes of moderate aerobic activity each week, along with strength training exercises.

- Monitor Changes: Keep track of your weight and any changes you notice after starting the pill. If you experience significant weight gain, consult your healthcare provider to discuss alternative contraceptive options.

6. Consulting a Healthcare Provider
If you have concerns about weight gain or other side effects related to birth control pills, it’s crucial to have an open discussion with your healthcare provider. They can provide personalized advice, help you weigh the benefits and risks of different contraceptive methods, and suggest alternatives if necessary.

In conclusion, while some women may experience weight changes when using birth control pills, it is not a universal outcome. Understanding the facts and myths surrounding this topic can help women make informed decisions about their contraceptive options. If you are considering starting or switching birth control methods, consulting with a healthcare professional is the best way to ensure that you choose the option that aligns with your health goals and lifestyle.
