Question
Hello Doctor: I have been taking birth control pills for 3 months to regulate my menstrual cycle.
The first three months my periods were normal, but this month (May), I have only about 2 pills left and I am experiencing abnormal bleeding.
I visited a gynecologist who said that since my period is approaching, they cannot prescribe me anything to stop the bleeding.
However, I have been bleeding continuously since May 24, and the amount is very light and irregular (previously, I had heavy menstrual flow).
What could be the issue and how should I address it? Additionally, I have been experiencing calf cramps for the past two days; could this be related? Please respond as soon as possible, thank you!
AMY, 20~29 year old female. Ask Date: 2005/06/02
Dr. Lü Lizheng reply Obstetrics and Gynecology
The response suggests that this may just be irregular menstruation, and treatment with menstrual regulation medication can be considered.
It is also important to consider the possibility of inflammation or pregnancy.
Sincerely, Lu Lizheng.

More Info
Irregular bleeding while on birth control can be concerning, especially if you have been using hormonal contraceptives to regulate your menstrual cycle. It’s important to understand that irregular bleeding, often referred to as "breakthrough bleeding," can occur for various reasons, even after a few months of stable use.
Firstly, hormonal contraceptives, including birth control pills, work by altering the hormonal balance in your body to prevent ovulation and regulate your menstrual cycle. However, during the initial months of use, your body may still be adjusting to these hormonal changes. This adjustment period can lead to irregular bleeding or spotting, which is common and usually not a cause for alarm.
In your case, since you have experienced consistent irregular bleeding for several days, it is essential to consider a few potential causes:
1. Hormonal Fluctuations: As mentioned, your body may still be adjusting to the hormones in the birth control pills. This adjustment can lead to changes in your menstrual cycle, including lighter periods or unexpected bleeding.
2. Missed Doses: If you have missed any doses of your birth control pill, this can lead to irregular bleeding. It’s crucial to take your pills consistently at the same time each day to maintain stable hormone levels.
3. Underlying Health Issues: Conditions such as polycystic ovary syndrome (PCOS), uterine fibroids, or endometriosis can also lead to irregular bleeding. If these conditions were present before starting birth control, they might still affect your cycle.
4. Infection or Other Medical Conditions: Infections, particularly sexually transmitted infections (STIs), can cause abnormal bleeding. Additionally, other medical conditions affecting the reproductive system may contribute to irregular bleeding.
5. Medication Interactions: If you are taking other medications, they may interact with your birth control pills, affecting their efficacy and leading to irregular bleeding.
Regarding your experience of leg cramps, while they are not directly related to irregular bleeding, they can be a side effect of hormonal contraceptives. Hormonal changes can affect fluid retention and muscle function, leading to cramps. However, if the cramps are severe or persistent, it is advisable to consult a healthcare provider to rule out any other underlying issues.
Recommendations:
1. Monitor Your Symptoms: Keep track of your bleeding patterns, including the duration and amount of bleeding. Note any other symptoms you may experience, such as pain or discomfort.
2. Consult Your Healthcare Provider: Since you have already seen a gynecologist, it may be beneficial to follow up if the irregular bleeding continues. They may recommend a different type of birth control or further evaluation to rule out any underlying conditions.
3. Consider Lifestyle Factors: Stress, diet, and exercise can all impact your menstrual cycle. Ensure you are maintaining a healthy lifestyle, which can help regulate your cycle.
4. Stay Hydrated and Manage Cramps: For leg cramps, staying hydrated and ensuring adequate intake of electrolytes may help. Gentle stretching and warm compresses can also alleviate muscle cramps.
5. Evaluate Your Birth Control Method: If irregular bleeding persists, discuss with your healthcare provider the possibility of switching to a different contraceptive method that may be better suited for your body.
In conclusion, while irregular bleeding on birth control can be common, it is essential to monitor your symptoms and consult with your healthcare provider for personalized advice and potential adjustments to your contraceptive method. Your health and comfort are paramount, and addressing these concerns with a professional can help ensure you find the best solution for your needs.
