Regarding issues after masturbation..?

Hello Dr.
Lin, since I developed the habit of masturbation during middle school, I used to do it about three times a day.
Now, I only do it about once or twice a week.
However, whenever I masturbate and the erection lasts for more than an hour, I experience pain in my abdomen and testicles.
Afterward, I also feel an itchy sensation in my kidneys.
Could this be what people commonly refer to as "kidney deficiency"? By the way, I previously injured my lower back during sports, and it still feels sore to this day.
Is there any connection between these issues?

JACK, 20~29 year old female. Ask Date: 2005/06/04

Hello Jack: "Kidney deficiency" is a term from Traditional Chinese Medicine, but what does it actually mean? As a member of Western medicine, particularly as a urologist, I do not agree with this concept.
Masturbation, in itself, does not lead to kidney failure; the kidneys do not have sensory nerve cells that can feel pain, so you cannot feel your kidneys in that way.
If you are experiencing soreness due to a back injury, please consult an orthopedic physician for treatment.

Understanding post-masturbation pain can be complex, especially when considering the various factors that may contribute to discomfort in the abdomen and testicles. Based on your description, it seems you experience significant pain during and after masturbation, particularly when the erection lasts longer than an hour. Additionally, you mentioned a sensation of itching in the kidney area post-masturbation and a history of lower back pain due to a previous injury.
Firstly, it’s important to clarify that the term "敗腎" (which translates to "defeated kidney") is not a medically recognized term in Western medicine. It is often used colloquially in some cultures to describe a perceived loss of vitality or health due to excessive sexual activity or masturbation. However, from a medical standpoint, there is no direct correlation between masturbation and kidney health.
The pain you experience in your abdomen and testicles could be attributed to several factors:
1. Prolonged Erection: When an erection lasts longer than normal, it can lead to a condition known as priapism, which is a prolonged and often painful erection. This can cause discomfort in the pelvic region, including the abdomen and testicles, due to increased blood flow and pressure in these areas.

2. Muscle Strain: Engaging in sexual activity, including masturbation, can involve various muscle groups. If you have a history of lower back pain, it’s possible that certain positions or the physical exertion involved in masturbation could exacerbate existing muscle strain or discomfort in your back, which may radiate to your abdomen.

3. Nerve Irritation: Given your history of back injury, there may be underlying nerve irritation or compression that could be contributing to the pain you feel during and after masturbation. Conditions such as a herniated disc or sciatica can lead to referred pain in the pelvic region.

4. Infection or Inflammation: Although less likely, it’s important to rule out any infections or inflammatory conditions affecting the urinary tract or reproductive organs. Conditions such as epididymitis (inflammation of the epididymis) or prostatitis (inflammation of the prostate) can cause pain in the testicles and lower abdomen.

5. Psychological Factors: Anxiety or stress related to sexual activity can also manifest as physical discomfort. If you have concerns about your sexual health or performance, this could contribute to the pain you experience.

Given these considerations, it is advisable to consult with a urologist. A urologist can perform a thorough evaluation, including a physical examination and possibly imaging studies, to determine if there are any underlying conditions that need to be addressed. They may also conduct tests to rule out infections or other medical issues.

In the meantime, here are some recommendations:
- Limit Duration: If you notice that prolonged erections lead to pain, consider limiting the duration of your erections through breaks or changing positions.

- Pain Management: Over-the-counter pain relief medications may help alleviate discomfort, but consult with a healthcare provider before taking any medication.

- Back Care: Since you have a history of back pain, consider physical therapy or exercises that strengthen your core and back muscles, which may help alleviate some discomfort during sexual activity.

- Hydration and Rest: Ensure you are well-hydrated and allow your body adequate rest, especially after engaging in physical activities.

In conclusion, while your symptoms may be concerning, they are not uncommon and can often be managed effectively with the right medical guidance. Seeking the advice of a urologist will provide you with the best course of action tailored to your specific situation.
