Removal of bone screws
Hello Dr.
Lee: How long should bone screws be left in before removal? I have had ankle screws for 7.8 years.
Should they be removed? If not removed, will it cause any issues? Thank you.

Hello: Bone screws are generally removed one year after surgery (depending on the fracture site, some may be removed in one year, others in one and a half years).
Leaving bone screws in the body without removal does not necessarily lead to complications; however, there are a few documented cases where individuals have suggested that prolonged presence of implants in the body may cause irritation in those with special sensitivities, potentially leading to tumors.
Additionally, some metal components of the implants can be absorbed by the body, resulting in elevated levels of heavy metals, which is not ideal.
This is why there is a search for more suitable materials, such as titanium or absorbable plastic materials.
Therefore, if the implants can be removed, it is advisable to do so whenever possible.
However, each case is unique and should be assessed based on imaging studies and the condition of the surrounding skin and soft tissues.
It is recommended that you consult with the original surgeon or a nearby orthopedic specialist to arrange for further management.
Wishing you happiness every day!

The decision to remove bone screws after surgery is influenced by several factors, including the type of fracture, the healing process, the patient's overall health, and the presence of any complications. Generally, bone screws and plates are used to stabilize fractures and allow for proper healing. Once the bone has sufficiently healed, the screws may be considered for removal, but this is not always necessary.
In your case, with the bone screws in your ankle for 7 to 8 years, it is important to evaluate whether they are still serving a purpose or if they are causing any discomfort or complications. Here are some considerations regarding the removal of bone screws:
1. Healing Time: Typically, bone healing can take anywhere from 6 months to a year, depending on the type of fracture and the individual’s healing capacity. In some cases, screws may be left in place indefinitely if they are not causing any issues. However, if the screws are still present after several years, it is essential to assess whether they are still necessary.
2. Symptoms and Complications: If you are experiencing pain, discomfort, or any other symptoms in the area where the screws are located, it may be a sign that they should be removed. Some patients report a sensation of the screws being a foreign body, which can lead to discomfort, especially during physical activity. Additionally, if there are signs of infection or irritation, removal may be warranted.
3. Risk of Complications: Leaving screws in place for an extended period can sometimes lead to complications, such as irritation of surrounding tissues, potential for the screws to loosen, or even the risk of fracture at the screw holes if significant stress is applied to the area.
4. Consultation with an Orthopedic Surgeon: It is crucial to have a thorough evaluation by an orthopedic surgeon who can assess your specific situation. They may perform imaging studies, such as X-rays, to determine the condition of the bone and screws. Based on this assessment, they can provide recommendations on whether removal is advisable.
5. Surgical Considerations: If it is determined that the screws should be removed, the procedure is generally straightforward and can often be done on an outpatient basis. Recovery time after screw removal is usually minimal, with most patients able to resume normal activities within a week or two, depending on individual healing and any underlying conditions.
6. Long-term Outcomes: Studies have shown that removing screws after a certain period can lead to improved comfort and function for many patients. However, some individuals may not experience significant changes after removal, especially if the screws were not causing any issues prior to the procedure.
In conclusion, while there is no strict timeline for when bone screws should be removed, it is essential to consider your symptoms, the duration the screws have been in place, and the advice of a qualified orthopedic surgeon. If you have concerns about your current situation, it is advisable to seek a consultation to discuss the potential benefits and risks of screw removal.
