Blisters on hands and feet with intense itching?
Dear Dr.
Jiang,
Recently, I have been experiencing small blisters on the backs of my fingers, between my fingers, and at the base of my fingers (as well as on my feet).
They are very itchy, and the itchiness is so intense that I feel like I need to scratch the blisters to relieve the itching.
However, I do not have any accompanying skin breakdown, redness, or swelling.
What could this issue be? Thank you for your response.

Dr. Jiang Zheen reply Dermatology
This is likely to be dyshidrotic eczema, which is often triggered by seasonal changes and manifests as hand eczema.
You can apply some topical corticosteroids for eczema to help alleviate the symptoms.
Wishing you well.

More Info
Itchy blisters on the hands and feet can be quite uncomfortable and concerning. Based on your description, it sounds like you may be experiencing a condition known as dyshidrotic eczema, also referred to as pompholyx. This skin condition is characterized by the sudden appearance of small, itchy blisters on the palms of the hands, fingers, and soles of the feet. The blisters can be filled with fluid and may cause significant itching, which can lead to scratching and potential secondary infections.
Causes of Dyshidrotic Eczema
The exact cause of dyshidrotic eczema is not fully understood, but several factors may contribute to its development:
1. Allergies: Some individuals may have an allergic reaction to certain substances, such as metals (like nickel), which can trigger the condition.
2. Stress: Emotional stress is known to exacerbate many skin conditions, including eczema.
3. Moisture: Excessive sweating or prolonged exposure to water can lead to the development of blisters.
4. Skin Irritants: Contact with irritants such as soaps, detergents, or chemicals can provoke an outbreak.
5. Genetics: A family history of eczema or other allergic conditions may increase the likelihood of developing dyshidrotic eczema.
Symptoms
The primary symptoms of dyshidrotic eczema include:
- Small, itchy blisters on the fingers, palms, and soles of the feet.
- The blisters may be filled with clear fluid and can be very itchy.
- The affected skin may become red or inflamed, especially if scratched.
- After the blisters dry up, the skin may peel and become dry or cracked.
Diagnosis
A healthcare provider can typically diagnose dyshidrotic eczema through a physical examination and by discussing your symptoms and medical history. In some cases, a patch test may be conducted to identify any potential allergens.
Treatment Options
While there is no cure for dyshidrotic eczema, several treatment options can help manage symptoms:
1. Topical Corticosteroids: These anti-inflammatory creams can help reduce itching and inflammation. Stronger prescription-strength corticosteroids may be necessary for severe cases.
2. Moisturizers: Regularly applying a thick moisturizer can help keep the skin hydrated and reduce the frequency of flare-ups.
3. Antihistamines: Oral antihistamines can help alleviate itching, especially at night.
4. Avoiding Triggers: Identifying and avoiding known triggers, such as certain soaps or stressors, can help prevent outbreaks.
5. Wet Dressings: Applying wet dressings to the affected areas can soothe the skin and help with healing.
6. Phototherapy: In some cases, light therapy may be recommended for chronic or severe cases.
When to See a Doctor
If your symptoms persist, worsen, or if you develop signs of infection (such as increased redness, swelling, or pus), it is important to seek medical attention. A dermatologist can provide a comprehensive evaluation and may recommend additional treatments, such as systemic medications for severe cases.
In conclusion, while itchy blisters on the hands and feet can be distressing, understanding the potential causes and treatment options can help you manage the condition effectively. If you suspect you have dyshidrotic eczema or if your symptoms are severe, consulting with a healthcare professional is the best course of action.
