Dizziness and tinnitus?

Hello Dr.
Hsu, I have been experiencing dizziness and tinnitus for two to three years.
I have seen quite a few doctors, but none have provided significant help for my condition.
A particularly unique phenomenon I experience is that I feel especially dizzy when I eat, especially with spicy foods.
Have you treated patients with similar symptoms? Also, since I live in Taipei City, do you know of any doctors in Taipei who have conducted substantial research on symptoms like mine?

Afu, 30~39 year old female. Ask Date: 2005/07/01

Hello: In response to your question, yes, it could be migraine and Meniere's disease, but the actual cause still needs to be evaluated in an outpatient setting.
There are such specialists in Taipei, such as Dr.
Lien Chiang-feng at Taipei Veterans General Hospital, Dr.
Yang Yi-ho at National Taiwan University Hospital, and Dr.
Wang Chih-hung at Tri-Service General Hospital.

Dizziness and tinnitus can be quite distressing symptoms, and it’s understandable that you are seeking further help, especially given the duration of your symptoms. Your experience of increased dizziness while eating, particularly with spicy foods, is indeed noteworthy and can be indicative of several underlying conditions.
Firstly, it’s important to recognize that dizziness can stem from various causes, including vestibular disorders, neurological issues, or even dietary triggers. In your case, the fact that your dizziness intensifies with certain foods may suggest a possible link to dietary sensitivities or even gastrointestinal issues that could be affecting your vestibular system. For instance, spicy foods can sometimes lead to increased acid production or reflux, which may in turn affect balance and cause dizziness.

Tinnitus, or ringing in the ears, is often associated with hearing loss, but it can also occur in conjunction with vestibular disorders. Conditions such as Meniere's disease, vestibular migraine, or even anxiety disorders can present with both tinnitus and dizziness. Given that you have seen multiple doctors without significant improvement, it may be beneficial to seek a specialist who has a particular focus on vestibular disorders or otolaryngology (ear, nose, and throat specialists).

In Taipei, there are several hospitals and clinics where you can find specialists who deal with dizziness and tinnitus. For instance, National Taiwan University Hospital and Taipei Veterans General Hospital have departments specializing in otolaryngology and neurology. You might want to look for doctors who have experience with vestibular rehabilitation or those who have published research on dizziness and tinnitus.
Additionally, it may be worthwhile to consider a comprehensive evaluation that includes not only a physical examination but also vestibular testing, audiometry, and possibly imaging studies like an MRI if deemed necessary by your physician. This thorough approach can help identify any underlying conditions that may not have been previously considered.

Moreover, if you have not already done so, keeping a symptom diary could be beneficial. Documenting when your dizziness occurs, the types of food you eat, and any other accompanying symptoms can provide valuable information to your healthcare provider. This can help in identifying patterns or triggers that may not be immediately obvious.

Lastly, if you suspect that your symptoms may be related to anxiety or stress, it could be beneficial to consult with a mental health professional. Anxiety can exacerbate physical symptoms, and addressing any underlying anxiety may help alleviate some of your dizziness and tinnitus.

In summary, while your symptoms are complex and have persisted for some time, seeking out a specialist in Taipei who focuses on vestibular disorders could provide you with the targeted care you need. A multidisciplinary approach that includes both medical and possibly psychological support may yield the best outcomes for your health.
