Bowel issues
Hello Doctor: Recently, I have been experiencing frequent diarrhea, with my stools appearing a clay-like yellow color, mixed with remnants of the food I ate in my last meal.
I have a history of constipation, where I would have bowel movements about 2 to 4 times a week, and at one point, I went 12 days without a bowel movement.
There was a period when I could hardly pass stool, and when I did, it was dark brown and in hard pieces.
I have researched some information, but I cannot determine the cause, and the literature states that bowel movements should be regular, but I do not have a fixed schedule.
Currently, my bowel movements are better than before (6 times a week), but they are all liquid.
This situation has persisted for about 3 weeks.
Additionally, I enjoy eating fruits and vegetables and exercise regularly, but I am not very fond of drinking water.
I also notice that if I drink cold milk in the morning, I will have diarrhea twice within an hour, but this does not happen at other times.
How can I establish a regular bowel movement schedule and ensure daily bowel movements? Do I need to see a doctor for my condition? If so, which specialty should I consult? Where would you recommend I go? Thank you for your response.

Hello, based on your situation, it seems that your bowel habits have changed.
Therefore, I recommend seeing a gastroenterologist.
If you have experienced constipation for more than five years, a colonoscopy is necessary.
Additionally, to promote daily bowel movements, it is essential to drink plenty of water and consume a high-fiber diet.
The colon's primary function is to absorb water (drinking beverages is ineffective, as they contain other ingredients that may hinder water absorption and have the opposite effect), so both are crucial.
Wishing you smooth bowel movements every day.
Sincerely, Dr.

More Info
Understanding bowel issues, particularly the transition between diarrhea and constipation, can be quite complex and often requires a multifaceted approach to treatment and management. Based on your description, it seems you are experiencing a significant fluctuation in bowel habits, which can be indicative of underlying gastrointestinal disorders such as Irritable Bowel Syndrome (IBS) or other functional gastrointestinal disorders.
Bowel Movement Patterns
Your symptoms of alternating diarrhea and constipation, along with the presence of liquid stools and changes in color, suggest a possible disruption in your gastrointestinal motility. The fact that you have a history of constipation, where you experienced infrequent bowel movements, and now have frequent liquid stools, indicates that your bowel function is not stable. The color of your stools, particularly the "土黃色" (earthy yellow), may suggest that food is moving too quickly through your intestines, preventing proper absorption of nutrients and water.
Possible Causes
1. Dietary Factors: While you mentioned a fondness for fruits and vegetables, the lack of adequate water intake can lead to dehydration, which can exacerbate constipation. Additionally, certain foods, especially dairy (like your experience with cold milk), can trigger gastrointestinal symptoms in some individuals, leading to diarrhea.
2. Irritable Bowel Syndrome (IBS): This is a common functional gastrointestinal disorder characterized by abdominal pain and altered bowel habits. IBS can manifest as diarrhea-predominant (IBS-D), constipation-predominant (IBS-C), or a mix of both (IBS-M). Stress and anxiety can also play a significant role in exacerbating symptoms.
3. Gastrointestinal Infections or Inflammation: Persistent diarrhea can sometimes be a sign of an infection or inflammation in the gastrointestinal tract. If you have had recent travel, dietary changes, or exposure to contaminated food or water, this could be a factor.
Recommendations for Management
1. Hydration: Increase your water intake. Aim for at least 8 glasses of water a day, especially if you consume a high-fiber diet. Proper hydration helps in forming bulkier stools and can alleviate constipation.
2. Dietary Adjustments: Consider keeping a food diary to identify any specific foods that may trigger your symptoms. You might want to limit dairy products if they cause diarrhea. Incorporate soluble fiber (found in oats, bananas, and apples) to help regulate bowel movements.
3. Regular Meal Schedule: Establishing a regular eating schedule can help train your body to have regular bowel movements. Try to eat meals at the same times each day.
4. Physical Activity: Continue your regular exercise routine, as physical activity can stimulate bowel function.
5. Stress Management: Since stress can exacerbate gastrointestinal symptoms, consider incorporating relaxation techniques such as yoga, meditation, or deep-breathing exercises into your routine.
When to See a Doctor
Given the persistence of your symptoms for about three weeks, it would be advisable to consult a healthcare professional. You should consider visiting a gastroenterologist, as they specialize in digestive system disorders. They can perform necessary evaluations, such as stool tests, blood tests, or imaging studies, to rule out any underlying conditions.
Conclusion
In summary, your bowel issues may stem from a combination of dietary habits, potential IBS, and possibly other gastrointestinal factors. By making some lifestyle adjustments and seeking medical advice, you can work towards achieving more regular and comfortable bowel movements. Remember, it’s essential to listen to your body and seek professional help when symptoms persist or worsen.
