Itching

Hello Dr.
Liao: Recently, I've been feeling itchy on my skin, as if mosquitoes or small insects are biting me.
My skin has become red from scratching, but I can't see or catch the insects that are causing the itchiness.
The itching is mild, but I find myself scratching throughout the day, which is quite frustrating and makes it difficult for me to concentrate on my studies.
Is it possible that I am too sensitive? What should I do to resolve this issue? I would greatly appreciate your advice.
Thank you!

Hongye, 20~29 year old female. Ask Date: 2001/06/21

Persistent itching can be a frustrating and distressing experience, often leading to significant discomfort and distraction from daily activities, such as studying or working. Understanding the underlying causes of your itching is crucial for effective management.
Firstly, it’s important to recognize that itching, or pruritus, can arise from various sources. Common causes include dry skin (xerosis), allergic reactions, insect bites, skin conditions such as eczema or psoriasis, and even systemic issues like liver or kidney disease. Given that you mentioned the sensation of being bitten by invisible insects, it could be indicative of several conditions, including:
1. Allergic Reactions: You may be experiencing an allergic reaction to something in your environment, such as dust mites, pet dander, or certain fabrics. Allergies can cause localized or generalized itching and redness.

2. Dry Skin: If your skin is dry, it can become itchy and irritated. This is especially common in colder climates or during winter months when humidity levels drop.

3. Dermatitis: Conditions like contact dermatitis can occur if your skin comes into contact with an irritant or allergen. This can lead to redness, swelling, and itching.

4. Insect Bites: Even if you don’t see the insects, it’s possible that you have been bitten by mosquitoes or other insects, which can cause localized itching.

5. Psychogenic Itching: Sometimes, stress or anxiety can manifest as itching, even in the absence of a physical cause.

To manage your persistent itching, consider the following strategies:
- Moisturize Regularly: Use a good quality moisturizer to keep your skin hydrated. Apply it immediately after bathing to lock in moisture. Look for products that are fragrance-free and designed for sensitive skin.

- Avoid Irritants: Identify and avoid any potential irritants. This includes harsh soaps, detergents, and fabrics that may cause irritation. Opt for mild, hypoallergenic products.

- Cool Compresses: Applying a cool, damp cloth to the itchy areas can provide immediate relief. It helps reduce inflammation and soothes the skin.

- Topical Treatments: Over-the-counter hydrocortisone cream can help reduce inflammation and itching. Antihistamines may also be beneficial if the itching is due to an allergic reaction.

- Bathing Practices: Take shorter baths or showers with lukewarm water instead of hot water, which can further dry out the skin. Adding colloidal oatmeal or baking soda to your bath can also help soothe itching.

- Wear Loose Clothing: Tight clothing can irritate the skin and exacerbate itching. Opt for loose-fitting, breathable fabrics.

- Consult a Dermatologist: If your symptoms persist despite these measures, it’s advisable to consult a dermatologist. They can conduct a thorough examination, possibly perform allergy testing, and recommend appropriate treatments tailored to your specific condition.

In summary, persistent itching can stem from various causes, and effective management often requires a multifaceted approach. By implementing these strategies and seeking professional advice when necessary, you can alleviate your discomfort and improve your quality of life. Remember, it’s essential to listen to your body and address any underlying issues that may be contributing to your symptoms.
