What are the treatments for plantar hyperhidrosis?

Hello Dr.
Jiang,
I would like to ask which treatment is more effective for foot sweating symptoms: iontophoresis or botulinum toxin injections? What is the approximate cost of iontophoresis treatment, and is the treatment process complicated?

Dr. Jiang Zheen reply Dermatology

Both methods have their pros and cons, but they are both symptomatic treatments.
The iontophoresis method requires several weeks of continuous sessions to achieve results; it is covered by health insurance, and you can also purchase a machine to use at home.
Botox injections can last for 4 to 6 months with a single treatment, but the downside is that we still do not know if there are any long-term side effects, and the cost is high.
Wishing you safety.

More Info

When it comes to treating excessive foot sweating, known medically as plantar hyperhidrosis, two common treatment options are iontophoresis and Botox (botulinum toxin). Both methods have their own advantages and disadvantages, and the choice between them often depends on individual circumstances, preferences, and the severity of the condition.

Iontophoresis is a non-invasive treatment that involves using a device to pass a mild electrical current through water and into the skin's surface. This process can temporarily block the sweat glands, reducing sweating in the treated areas. Many patients find iontophoresis effective, especially for localized sweating such as in the hands and feet.
The treatment typically requires multiple sessions, often two to three times a week initially, followed by maintenance sessions every few weeks or months. The cost of iontophoresis can vary widely depending on the clinic and location, but it is generally more affordable than Botox treatments. In some cases, insurance may cover part of the cost, especially if the treatment is deemed medically necessary. One of the advantages of iontophoresis is that patients can purchase a machine for home use, allowing for ongoing treatment without frequent clinic visits.

On the other hand, Botox injections work by blocking the nerves that trigger sweating. The effects of Botox can last anywhere from four to six months, making it a more convenient option for some patients who prefer fewer treatment sessions. However, Botox is typically more expensive than iontophoresis, and the cost can add up over time since repeat injections are necessary to maintain the effects. Additionally, while Botox is generally safe, there are potential side effects, including pain at the injection site, temporary weakness in nearby muscles, and the possibility of developing antibodies that could reduce the effectiveness of future treatments.

In terms of effectiveness, studies have shown that both treatments can significantly reduce sweating, but individual responses can vary. Some patients may find that iontophoresis works well for them, while others may prefer the convenience of Botox. It is essential to consult with a healthcare provider to discuss the best option based on personal medical history, the severity of sweating, and lifestyle considerations.

In conclusion, both iontophoresis and Botox are viable options for treating foot sweating, each with its own set of benefits and drawbacks. Iontophoresis may be more suitable for those looking for a cost-effective and non-invasive solution, while Botox may appeal to those seeking a more long-term and less frequent treatment regimen. Ultimately, the decision should be made in consultation with a healthcare professional who can provide personalized recommendations based on the patient's specific needs and circumstances.
