Pain in the testicles and the area above the base of the penis?

Hello Doctor,
I would like to ask you about something.
I recently started dating a girl after not having interacted with women for two years.
Whenever we hold hands or hug, I often feel quite wet down there, but I find it difficult to achieve an erection, which makes me worried that I might have a sexual dysfunction.

When we first met, there was an occasion when we hugged for about two hours at the beach, and afterward, I started to feel pain in my testicles, followed by pain in my bladder.
When I went to bed that night, the pain was so intense that I could hardly move.
The next day, the pain persisted from noon until evening.
I went to see a doctor who examined me and conducted a urinalysis, which came back normal.
The doctor mentioned that my testicles were somewhat droopy and administered an injection and prescribed some medication.
However, I feel that the issue might not be related to that, as I know I don't experience any problems when I'm not with a woman.
Additionally, today I noticed that during foreplay with my girlfriend (we did not engage in sexual intercourse), my penis was unable to achieve an erection.
After some time, I began to feel pain again, particularly in the area around my testicles and just above the base of my penis.
Later, when I returned home, I attempted to masturbate to ejaculate, but I found it difficult to achieve an erection, and when I did, the hardness was not satisfactory.
I suspect that the pain might be causing the difficulty in getting an erection, as I can still achieve a normal erection during masturbation.
Could you please advise me on what might be happening? Thank you.
Why is it difficult to achieve an erection? Why is there pain in the testicles and above the base of the penis?

The issue needs clarification; it is recommended that you visit the outpatient clinic for a physical examination to determine the problem.

It sounds like you are experiencing a combination of physical discomfort and psychological stress related to your sexual health. Let's break down the issues you're facing and explore potential causes and solutions.

Firstly, the pain in your testicles and the area around the base of your penis can be concerning. Testicular pain can arise from various causes, including trauma, infection, or conditions like epididymitis (inflammation of the epididymis) or testicular torsion. Since you mentioned that the pain seems to occur after physical intimacy, it could also be related to muscle tension or strain in the pelvic area, especially if you are not used to physical contact after a long period of abstinence.

The fact that you experienced significant pain after hugging for an extended period suggests that your body may be reacting to the physical stimulation in a way that is causing discomfort. This could be due to a combination of factors, including anxiety about sexual performance, physical tension, or even a psychosomatic response where your mind is influencing your physical sensations.

Regarding your difficulty achieving an erection, this can be attributed to several factors. Psychological factors such as anxiety, stress, or fear of performance can significantly impact erectile function. Since you mentioned that you have not been in contact with women for two years, it is possible that you are feeling anxious about your sexual performance, which can lead to difficulties in achieving or maintaining an erection. This is often referred to as "performance anxiety," and it is a common issue among men, especially when they are in new relationships.

It is also worth noting that physical health plays a crucial role in sexual function. Factors such as hormonal imbalances, circulation issues, or even certain medications can affect your ability to achieve an erection. Since you mentioned that you can achieve an erection during masturbation but struggle during intimate moments with your girlfriend, this further points to a psychological component rather than a purely physical one.

Here are some steps you can take to address your concerns:
1. Consult a Healthcare Professional: Since you have already seen a doctor, it may be beneficial to follow up with a urologist or a sexual health specialist. They can conduct a thorough examination and possibly run tests to rule out any underlying medical conditions.

2. Manage Anxiety: Consider speaking with a mental health professional who specializes in sexual health or anxiety. They can help you develop coping strategies to manage performance anxiety and improve your confidence in intimate situations.

3. Communicate with Your Partner: Open communication with your girlfriend about your feelings and experiences can help alleviate some of the pressure you may be feeling. A supportive partner can make a significant difference in reducing anxiety.

4. Practice Relaxation Techniques: Engaging in relaxation techniques such as deep breathing, meditation, or yoga can help reduce overall stress and tension in your body, which may improve your sexual function.

5. Gradual Exposure: If you feel comfortable, try to gradually increase physical intimacy with your girlfriend in a low-pressure environment. This can help you become more accustomed to physical contact without the added stress of performance expectations.

6. Physical Health: Ensure you are maintaining a healthy lifestyle, including regular exercise, a balanced diet, and adequate sleep. These factors can all contribute to better sexual health.

In summary, your symptoms may be a combination of physical discomfort and psychological factors. It is essential to address both aspects to improve your sexual health and overall well-being. Seeking professional help and communicating openly with your partner can be significant steps toward resolving these issues.
