Painful engorgement of the penile blood vessels?

I am 32 years old and married.
A few days ago, I started noticing that the veins on my penis become particularly hard and prominent when I have an erection.
Pressing on the raised veins is painful.
Initially, only one vein on the right side exhibited this symptom, but in the past few days, I have also noticed a ring-like vein appearing under the glans, which is also painful when pressed.
Normally, there is no pain during an erection, only when I press on the raised veins.
What could this condition be? How should I address it? Thank you.

Mr. Zhang, 30~39 year old female. Ask Date: 2005/08/02

Dr. Xu Weikai reply Urology

Mr.
Zhang: Hello, based on your description, it seems that the blood vessels and lymphatic vessels on the penis may be obstructed, leading to gradual swelling, which is mostly caused by a local infection.
For a detailed diagnosis, it is best to seek medical attention nearby from a urologist or dermatologist.
Sincerely, Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

Experiencing painful veins during an erection can be concerning, and it's important to understand the potential causes and treatment options available. Based on your description, it seems that you are experiencing discomfort in the veins of the penis, particularly during an erection. This condition can be attributed to several factors, and I will outline some possible causes and recommendations for management.

Possible Causes
1. Venous Engorgement: During an erection, blood flow to the penis increases significantly, leading to the engorgement of the veins. If the veins are particularly prominent and painful, it may indicate that they are under increased pressure or that there is some form of inflammation.

2. Peyronie's Disease: This condition involves the development of fibrous scar tissue inside the penis, which can cause curvature and pain during erections. While you mentioned that the pain is localized to the veins, it’s worth considering if there are any other symptoms, such as curvature or changes in the shape of the penis.

3. Thrombophlebitis: This is the inflammation of a vein due to a blood clot, which can cause pain and tenderness. If the veins are swollen and painful, this could be a possibility, especially if there are risk factors such as prolonged immobility or recent trauma.

4. Varicose Veins: Just as in other parts of the body, veins in the penis can become varicosed, leading to discomfort and pain. This condition is usually benign but can cause significant discomfort.

5. Infection or Inflammation: Conditions such as sexually transmitted infections (STIs) or other inflammatory conditions can cause pain and swelling in the genital area, including the veins.

Recommended Actions
1. Consult a Healthcare Provider: Given that you are experiencing pain, it is essential to consult with a healthcare provider, preferably a urologist. They can perform a physical examination and may recommend imaging studies, such as an ultrasound, to assess the blood flow and structure of the veins.

2. Avoid Strain: Until you have a clearer understanding of the cause, it may be wise to avoid activities that could exacerbate the pain, such as vigorous sexual activity or heavy lifting.

3. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, may help alleviate discomfort. However, consult your doctor before taking any medication, especially if you have underlying health conditions or are taking other medications.

4. Monitor Symptoms: Keep track of any changes in symptoms, such as increased pain, swelling, or changes in erectile function. This information will be valuable for your healthcare provider.

5. Lifestyle Modifications: Maintaining a healthy lifestyle, including regular exercise, a balanced diet, and avoiding smoking, can improve vascular health and may help prevent similar issues in the future.

6. Follow-Up: If the pain persists or worsens, or if you notice other symptoms such as changes in urination, discharge, or erectile dysfunction, seek medical attention promptly.

Conclusion
Painful veins during an erection can arise from various causes, ranging from benign to more serious conditions. It is crucial to seek medical advice to determine the underlying issue and receive appropriate treatment. Early intervention can help prevent complications and ensure that you maintain your sexual health and overall well-being.
