Hypersensitive skin..?
Hello, Dr.
Huang.
I have a question for you.
My skin has been "extremely" sensitive since elementary school.
Whenever I lightly scratch my skin with my fingernail, within a minute, the scratched area swells up as if it has been bitten by a mosquito, although I don't experience any other sensations.
For example, if I gently write the character "大" on my skin with my fingernail, it will actually "appear" as a raised mark on my skin shortly after.
What could be the reason for this? I have previously consulted a dermatologist, but I haven't found a solution.
Although it doesn't hurt, it is quite bothersome to me.
Can you provide me with an answer?

This is called "dermatographism," which is a type of urticaria.
It can be managed with medication.

More Info
Extreme skin sensitivity can be a perplexing and frustrating condition, as you've described in your question. The symptoms you are experiencing, such as the skin reacting dramatically to light pressure or scratching, are indicative of a condition known as "dermatographism" or "skin writing." This is a form of physical urticaria (hives) where the skin becomes raised and inflamed in response to minor trauma, such as scratching or pressure.
Causes of Extreme Skin Sensitivity
1. Dermatographism: This condition occurs when the skin reacts to physical stimuli. It is thought to be due to an abnormal release of histamine from mast cells in the skin, leading to localized swelling and redness. The exact cause of dermatographism is not fully understood, but it may be associated with other allergic conditions or may occur without any underlying disease.
2. Genetic Factors: Some individuals may have a genetic predisposition to sensitive skin or conditions like dermatographism. If you have a family history of skin sensitivities or allergies, this could be a contributing factor.
3. Environmental Factors: Changes in temperature, humidity, or exposure to irritants (such as soaps, detergents, or fabrics) can exacerbate skin sensitivity.
4. Underlying Conditions: Conditions such as eczema, psoriasis, or other forms of dermatitis can make the skin more reactive. If you have a history of skin allergies or sensitivities, this could also play a role.
5. Stress and Anxiety: Psychological factors can also influence skin sensitivity. Stress can lead to increased histamine release, which may exacerbate symptoms.
Solutions and Management Strategies
1. Avoid Triggers: Identifying and avoiding triggers that exacerbate your skin sensitivity is crucial. This may include avoiding tight clothing, harsh soaps, or extreme temperatures.
2. Moisturization: Keeping the skin well-hydrated can help reduce sensitivity. Use fragrance-free, hypoallergenic moisturizers to maintain skin barrier function.
3. Antihistamines: Over-the-counter antihistamines may help reduce the histamine response in your skin. Consult with a healthcare provider to determine the best option for you.
4. Topical Treatments: Corticosteroid creams may be prescribed by a dermatologist to reduce inflammation and itching in affected areas.
5. Stress Management: Techniques such as mindfulness, meditation, or yoga can help manage stress, which may, in turn, reduce skin sensitivity.
6. Consult a Dermatologist: Since you have already seen a dermatologist without resolution, consider seeking a second opinion or asking for a referral to a specialist in allergic or immunological skin conditions. They may conduct tests to rule out other underlying issues.
7. Patch Testing: If you suspect that certain products or materials are causing your skin reactions, patch testing can help identify specific allergens.
8. Lifestyle Changes: Maintaining a healthy lifestyle, including a balanced diet, regular exercise, and adequate hydration, can improve overall skin health.
Conclusion
Extreme skin sensitivity can be a challenging condition to manage, but understanding its potential causes and implementing effective strategies can help alleviate symptoms. It's essential to work closely with a healthcare provider to develop a personalized management plan tailored to your specific needs. If your symptoms persist or worsen, further evaluation may be necessary to explore other underlying conditions that could be contributing to your skin's sensitivity.
