Gastrointestinal issues?
Hello Dr.
Zhao, I have been experiencing gastrointestinal issues for quite some time, but I have never seen a doctor.
I have many questions and would like to know if having bloating and unusual sounds from my stomach (similar to hunger growls or gurgling sounds) along with occasional burping—though not the kind that comes after a meal, but rather a sensation of gas trapped in my throat or chest that is difficult to expel—could indicate digestive problems.
When I do manage to burp, it usually comes in clusters of three or four.
Sometimes, when I eat or drink, it feels like gas is blocking my throat, which makes me wonder if this could lead to indigestion.
(I also feel the need to pass gas when I have a lot of gas in my stomach.) When I lie down and press on my abdomen, I can hear gurgling sounds (usually occurring a few hours after drinking water), and it feels like my stomach is filled with water and gas.
Occasionally, I drink milk (or milk tea) or probiotic drinks like Yakult one to two hours before bed, and I notice that I have mild diarrhea the next morning.
Could this be due to poor absorption in my gastrointestinal tract? Even more concerning, sometimes after drinking a lot of water (or beverages), I feel the urge to use the restroom, and what comes out is almost entirely liquid, as if my gastrointestinal tract did not absorb anything and it just passed straight through.
Sometimes, the stool contains vegetable remnants, which I find quite unpleasant.
Is this also a sign of poor absorption? Additionally, when I mentioned having gas in my stomach, drinking water makes it feel cool (I’m not sure if this is due to the water coming into contact with the gas).
I also engage in modeling and often inhale the fumes from the paint (the paint contains water, synthetic resin, organic solvents, and pigments).
I wonder if my gastrointestinal discomfort could be related to inhaling too many toxic fumes, although I always wear a mask and my room is well-ventilated.
These issues have been troubling me for a long time, and they seem to have started when I entered high school.
However, I don’t feel stressed; I lead a happy life and enjoy exercising.
I would greatly appreciate it if you could provide detailed explanations regarding my concerns.
Thank you very much, and I extend my highest respect!

Hello! Your symptoms are not indicative of malabsorption; otherwise, you would have experienced nutritional weight loss by now.
These symptoms are consistent with Irritable Bowel Syndrome (IBS), which is actually a quite common condition—about one in ten patients in a gastroenterology clinic has this disorder.
Water does not directly reach the colon upon consumption; rather, the act of drinking water stimulates the release of water retained in the colon.
Dizziness is also a common complaint among these patients, but it is unrelated to the inhalation of paint fumes.
This condition is caused by either rapid intestinal motility or heightened visceral sensitivity, and it is not associated with psychological stress, although prolonged symptoms may lead to mood changes.

Hello! Thank you for sharing your concerns regarding your gastrointestinal issues. It sounds like you are experiencing a complex array of symptoms that can be quite distressing. Let’s break down your symptoms, potential causes, and some solutions.
From what you've described, you are experiencing bloating, unusual sounds from your abdomen, difficulty burping, and sensations of gas trapped in your throat or chest. Additionally, you mentioned that you sometimes have loose stools, especially after consuming dairy products or drinks high in sugar. These symptoms can be indicative of several gastrointestinal conditions, including functional gastrointestinal disorders like Irritable Bowel Syndrome (IBS), lactose intolerance, or even food intolerances.
Symptoms Breakdown
1. Bloating and Gas: This is often caused by the fermentation of undigested food in the intestines, leading to gas production. Foods that are high in fiber, certain carbohydrates, and dairy products can contribute to this. You mentioned that you sometimes feel like you have gas trapped in your throat, which can be uncomfortable and may lead to excessive burping.
2. Unusual Sounds: The gurgling or "water sounds" you hear from your abdomen are typically normal and can occur due to the movement of gas and fluids in the intestines. However, if these sounds are accompanied by discomfort, it may indicate that your digestive system is struggling to process what you eat.
3. Loose Stools: The occurrence of loose stools, especially after consuming dairy or sugary drinks, might suggest lactose intolerance or a sensitivity to certain sugars (like fructose). If your body lacks the necessary enzymes to break down these substances, it can lead to diarrhea.
4. Feeling of Fullness: If you feel full quickly or experience discomfort after eating, it could be related to how your body is processing food. This sensation can also be a sign of delayed gastric emptying or other motility issues.
Potential Causes
- Irritable Bowel Syndrome (IBS): This is a common condition that affects the large intestine and can cause symptoms like bloating, gas, diarrhea, and constipation. Stress and diet often play significant roles in IBS.
- Lactose Intolerance: If you notice that consuming dairy products leads to gastrointestinal distress, you might be lactose intolerant. This means your body does not produce enough lactase, the enzyme needed to digest lactose.
- Food Intolerances: Besides lactose, other food intolerances (such as gluten or certain carbohydrates) can lead to similar symptoms.
- Gastroesophageal Reflux Disease (GERD): The sensation of gas trapped in your throat could also be related to GERD, where stomach acid flows back into the esophagus, causing discomfort.
- Dietary Habits: Consuming large amounts of sugary drinks or high-fat foods can exacerbate gastrointestinal symptoms. Additionally, drinking fluids right before or during meals can sometimes lead to a feeling of fullness or discomfort.
Recommendations
1. Dietary Adjustments: Consider keeping a food diary to track what you eat and how it affects your symptoms. This can help identify any specific triggers. You might want to reduce your intake of dairy products, high-sugar beverages, and foods that are known to cause gas (like beans, lentils, and certain vegetables).
2. Hydration: While staying hydrated is essential, try to drink fluids between meals rather than during meals to avoid feeling overly full.
3. Probiotics: Incorporating probiotics into your diet may help improve gut health and digestion. Foods like yogurt, kefir, and fermented vegetables can be beneficial.
4. Consult a Healthcare Professional: Given the complexity of your symptoms, it would be wise to consult a gastroenterologist. They may recommend tests such as lactose intolerance tests, stool tests, or even an endoscopy if necessary.
5. Avoid Inhalation of Harmful Substances: While you mentioned wearing a mask while working with model paints, it’s crucial to ensure that you are in a well-ventilated area to minimize exposure to any harmful fumes, as this could potentially affect your overall health.
6. Stress Management: Although you mentioned that you don’t feel stressed, incorporating relaxation techniques such as yoga or meditation can be beneficial for overall gut health.
In conclusion, while your symptoms can be distressing, many gastrointestinal issues can be managed effectively with dietary changes and medical guidance. It’s essential to listen to your body and seek professional help to ensure that you receive the appropriate care. Thank you for reaching out, and I hope this information helps you on your journey to better digestive health!
